EVALUATION PRE-OBSERVATION DATA SHEET

This is a working document to be:

1. completed by the employee, or

2. discussed between the employee and evaluator

at least one working day prior to each formal evaluation observation. This document does not become part of
the permanent record.

1. Describe any general background information you believe would be beneficial for the evaluator’s
understanding of what he/she will observe in this class. This may include information concerning the
nature of the course, students, etc.

    

2. Briefly describe where this lesson falls in the instructional sequence (what happened before, what will
happen after) and the general objectives of this lesson.

     

3. Describe the teaching methods and learning activities which may be observed.
