SUBECT: Authorizarion letter in order to withdraw money

Dear Sir / Madam,

I Ma. Donna Benito writing regarding my BPI Account number 8379366466 that I have with
your institution. I need to withdraw 15,000 pesos from my account. Furthermore, if you can do
me a favor if I can access through BPI Mobile since I don't have username and password.

I have this account as working as field Architect in Makati Development Corporation since last
year 2019. However, due to enhance community quarantine that had been implemented on
our country. I therefore hereby authorize my father, Domindaor Benito, be appointed
representative to withdraw the above mentioned from my account during the lockdown
downtime.

I hope this letter of authorization will prove the authenticity of my father as my representative
