ASSOCIATE COMMUNICATION

BluMine Health® Clinic

Attention KACI Associates,

We need your help in supporting our efforts to comply with CDC recommended protocols. We’ve had a couple of folks
come to the clinic as walk-ins and we are happy to take care of everyone but we need to follow our prescreening
requirements listed below even when you have no flu symptoms. We want to protect those not infected and ensure
we can continue to serve you and our community. Please call the clinic for an appointment. Thank you so much for
your help!

With the continued news on the Coronavirus and the updates from the CDC and other local health authorities on how
it should be handled, BluMine Health® has decided to implement an additional layer of protection for both you, our
beloved clients and patients, and the BluMine® staff that we value so highly. Our first and primary obligation is to every
patient that we treat and we want to treat all people with the same respect and care we would want our own families
to be given, as well as providing the safest atmosphere for everyone. In anticipation of this new procedure, all
providers will be contacting their health department for instructions on how to handle questions or positive screens for
the Coronavirus. Our understanding is that local health departments cannot test for the Coronavirus.

As patients call in or are scheduled, BluMine® will be asking the following questions:

• Are you experiencing a fever?

• Are you experiencing a cough?
• How long have you been experiencing cold/flu like symptoms?
• Have you traveled outside the US within the past 2-3 weeks?
• Have you been exposed to anyone who has the coronavirus or may have been exposed to the coronavirus?
• What date(s) were you exposed to that person?

If the provider has a positive pre-screen, we will provide the best care option possible, along with the instructions
provided by the health department.

BluMine® asks for your support regarding CDC recommendations in preventing walk in patients to our clinics. We
appreciate your compliance and ask that you call ahead to be seen. We have posted a sign on the clinic door to remind
patients as well. We truly appreciate your help in ensuring adherence to this protocols.
