Complication of pace maker

Pocket complications
 Pocket hematoma, Infection, Erosion, Wound pain, Allergic reactions
 Pneumothorax /air embolism, Cardiac perforation
 Venous thrombosis, Coronary sinus dissection
 Lead dislodgement, Twidller syndrome
Pacemaker malfunction
1. Pacemaker syndrome: fatigue, dizziness and hypotension.
2. Abnormal Pacemaker function
a. Failure to Pace
b. Failure to Sense
c. Failure to Capture
d. Over sensing
3. Pacemaker Mediated Tachycardia

Midline sternotomy scar


 Aortic Valve Replacement
 Mitral valve Replacement
 CABG
 Heart lung transplantation
 Open Mitral Valvotomy
 Total correction of Fallots
 Mediatinal surgery: eshophagectomy, Thymectomy, retrosternal goiter
Left thoracotomy scar
 MVR, Mitral Valvotomy
 Coarctation repair, Blalock - Taussig Shunt for Fallots
Left or right infraclavicular scar
 Permanent Pacemaker (PPM)
 Automatic Implantable Cardioverter Defibrillator (AICD)
Midline sternotomy scar + lateral thoracotomy scar
There are 4 differentials:
1. Coarctation repair
2. TOF repair
3. VSD repair
4. Metallic mvr with previous valvoplasty

Anterior cubital fossa: angiography


Scars seen in chron’s:
1. Midline laparotomy scar.
2. Iliostomy scar.
3. PEG tube scar.

Parathyroidecromy scar:

In chest lateral thoracotomy scar seen in :


*Pneumonectomy and lobectomy.
*Single lung transplantation.

If you suspect that pulmonary fibrosis is on the differential diagnosis, then the patient
may have undergone a VATS lung biopsy to get a tissue diagnosis. Also used for pleural
biopsies, decortication, pleurodesis.
Mediastinoscopy scars:
Mediastinoscopy scar : A horizontal scar about 1 cm above the sternal notch, very easily
mistaken for a tracheostomy scar. Used for the investigation of mediastinal
lymphadenopathy. e.g. Sarcoid, TB, lymphoma, staging of primary lung cancer
