Assessment Scale:
- See attached grading rubric
Subject Matter/Content: Science
Prerequisites:
• Students will already be able to identify main components of the heart.
• Students will be familiar with foldables and how to fill it out.
• Heart healthy nutrition from previous day
Key Vocabulary:
These words will be written on the board for the students to reference
• Inflammation: condition in which part of the body becomes reddened, swollen, hot,
and often painful, especially as a reaction to injury or infection.
• Heart Disease: Means there are problems with the heart and blood vessels.
• Blood Vessels: a tube that carries blood in the circulatory system
• Arteries: a blood vessel that carries blood away from the heart to other parts of the
body.
• Blood Pressure: measures how hard the blood is pushing against the walls of the
arteries.
• Plaque: a sticky usually colorless thin film on the teeth that is formed by and contains
bacteria.

Content/Facts:
• Poor eating habits-Eating too much foods high in sugar and sodium is bad for the heart.
Poor eating can result in heart disease.
• Smoking- Smoking can damage the function of your heart and blood vessels. Smoking
creates plaque buildup in the arteries which can lead to heart disease.
• Lack of exercise- Inactivity can lead to high blood pressure and damages the wall of
the blood vessels.
• Not Flossing- by not flossing, plaque can build up in your mouth and cause gum
disease. This can cause inflammation and increased risk of heart disease.

Introduction/Activating/Launch Strategies:
• “The other day, we saw a picture of a healthy and unhealthy heart and we learned
about what makes a heart healthy”.
• “Today we are going to learn about what makes a heart unhealthy.
• “On the board, there is a picture of a heart with a sad face that is surrounded by smoke.
There also is a picture of a real damaged heart. With a partner, think of what these two
pictures have in common and what can be causing the heart to look that way.
• Give the students three minutes to talk and them bring the class together.
• Have two students share what they think.
• “Yes, both of these hearts are damaged, and we’ll learn more about why they are
damaged.

• Development/Teaching Approaches

Model:

• Before you start the model make sure the “What can cause an unhealthy heart?” sheet
is printed out and cut along the doted lines.
• Set the desks up into 4 different stations in each corner of the room with an
iPad/computer at each station.
• Set up the videos with the corresponding video and paper you cut out.
-”Poor eating habits-Eating too much foods high in sugar and sodium is bad for the
heart. Poor eating can result in heart disease.” Paper paired with
https://www.youtube.com/watch?v=5iJcLl0sHic video on iPad/computer
-“Smoking- Smoking can damage the function of your heart and blood vessels.
Smoking creates plaque buildup in the arteries which can lead to heart disease.” Paper
paired with https://www.youtube.com/watch?v=hCC1dgmwO_4 video on
iPad/computer.
-“Lack of exercise- Inactivity can lead to high blood pressure and damages the wall of
the blood vessels.” Paper paired with
https://www.youtube.com/watch?v=i70k0xFWILQ video on iPad/computer.
- “Not Flossing- by not flossing, plaque can build up in your mouth and cause gum
disease. This can cause inflammation and increased risk of heart disease.” Paper
paired with https://www.youtube.com/watch?v=u8iSDZBgBuw video on
iPad/computer.
• “Now we are going to brainstorm ways we think the heart can become unhealthy.
Remember about the heart surrounded by the smoke.”
• Give the students 2 minutes to come up with ideas
• Have at least three students share what they came up with and write it on the board.
• “ As everyone saw when they came into class today, we have iPads on some of the
desks. Today we are going to rotate around the room in groups and watch different
short video about ways the heart can become unhealthy.”
• “ I know using the iPads is very exciting for us, but we have to remember to be careful
around them.”
• “I will be giving each of you a foldable that you will fill out at each station.”
• “The stations all have a piece of paper for you to copy the information into your
foldable.”
• “I want you to copy the information down first and then I want your group to watch the
video. Remember to pay attention to the video and think about how the video relates to
the information you wrote down in the foldable.”
• Put the students into 4 groups of 5 and have each group start at different station.
• “Each video is pretty short so your group will have 6 minutes at each station.”
• “Does anyone have any questions?”
• “I will be walking around the class while you work, so make sure you ask me questions
and stay on task”.
• Start the groups on the stations and walk around the class to make sure the students are
understanding the information.
• After the students complete their foldable, have the groups come together.
• “ Can 4 different people tell me what each station was about?”
• Call on 4 students.
• “Yes, we learned that lack of exercise, smoking, not flossing, and poor eating habits
are all causing of an unhealthy heart.”

Guided Practice:
• “We just learned so much about the causes of an unhealthy heart.”
• “Now we are going to see what everyone remembers about unhealthy hearts.”
• Hand out a crossword puzzle to each student and give them 10 minutes to complete it.
• Walk around the room to check on the students and make sure they are on task.
• After the 10 minutes, put the answer key on the projector and as you read each
question, ask the students to volunteer and give the class the answer.
Independent practice:
• “We have been learning how important nutrition is for our bodies and how what you
eat effects your heart the past few days”.
• “We learned a lot about MyPlate yesterday and today we learned that eating unhealthy
foods can lead to an unhealthy heart.”
• “In the front of the room there are a lot of grocery flyers.”
• “ I’m going to hand out a worksheet and your job is to find and cut out 4 unhealthy
foods and 4 healthy foods from the flyers and glue it to the hearts.”
• Pass out the flyers and worksheets to each student.
• “Make sure you paste the unhealthy food on the unhealthy heart and the healthy food
on the healthy heart.
• “Also make sure you answer the “fill in” section on the bottom of the sheet.”
• After each student is done, collect their work and assess it based on the rubric.
Closure/Summarizing Strategies:
• “Over the past week, we have learned about the main components of the heart and
how important it is to function properly.”
• “We learned ways to keep our heart healthy and we learned about how important
nutrition is for our heart.”
• “And today we finished our heart unit on ways the heart can become unhealthy”
• “With a partner, I want you to talk about how important it is to maintain a healthy heart
and why its important to know how the heart functions.”
• Have at least three people share what they talked about.

Accommodations/Differentiation:
• To support MJ, the teacher can provide him with notes that are already filled in since
he has a fine motor impairment.
