Step 1
Fill-up College Entrance Exam Application Form

 P20.00 (testing fee) at accounting window 1.


 Proceed to TESTING ROOM for examination.
 After the exam, wait for the results.Step 2
Fill-up CLEARANCE FORM the proceed to ICCT Clinic for physical checkup.

Step 3
Submit Clearance Form along with the following documents:

 ORIGINAL COPY OF HIGH SCHOOL CARD

 ORIGINAL COPY OF GOOD MORAL CERTIFICATE

 2 COPIES OF DIPLOMA (Xerox)

 2 COPIES OF NSO BIRTH CERTFICATE (Xerox)

 4 PCS. 2X2 COLORED PICTURES

Step 4
Proceed to Registrar Window 1 and get tentative schedule/ subjects.

Step 5
Proceed to Course Adviser (Degree/ Non-Degree) at the Gym for checking of

Have your Clearance Form signed at the following offi ces starting on:

 Clinic

 Accounting Offi ce (Window 7)


Step 2
Get Number Stub at the Main Lobby for releasing of Grade Slip.

Step 3
Claim your Grade Slip & Tentative Form at the Registrar’s offi ce (Window 1 / 2)

Step 4
Proceed in the GYM to get your Schedule & Subjects. Fill-up your Tentative Form.

Step 5
Proceed to Course Adviser (Degree/ Non-Degree) at the Gym for checking of
Schedule and Subjects. Bring Tetative Form & Grade Slip.

Step 6
Get the Number Stub at the Main Lobby for Registrar’s Encoding of Schedule and
Subjects.

Step 7
Lastly, get a number stub and go to Accounting Offi ce for the following:

 Payment of Fees

 Releasing of Registration Form & Official Receipt.

 ID Validation
 Options to pay at PBB (Philippine Business Bank) or UCPB (United
Coconunt Planters Bank) after encoding schedules & subjects.
Step 8
Get schedule for drug testing at the Drug Testing Center.
