SWORN STATEMENT

I, Edgardo P. Perez, of legal age, married, Filipino, and a single proprietor/General

Manager of the South East Star Enterprises with business address at Apitong Ave., Mt. View
Subd. Poblacion 3, Tanauan City, after having been duly sworn to in accordance with law, do
hereby depose and say:

1. That I executed a Special Power of Attorney in favor of Audrey Ann C. Landicho, of

legal age, employee of my establishment, and residing at Brgy. Altura Bata, Tanauan
City, copy of which was already attached in the case filed by South East Star
Enterprises against Medelyn Blanco docketed in the Office of the Tanauan City
Prosecutor as NPS-IV-21-INV-10J-00141.

2. That in the authority therein stated in the aforesaid Special Power of Attorney, it
includes her right to withdraw said case against respondent Medelyn Blanco.

3. That I executed this Affidavit to attest to the fact herein above stated.
