AFFIDAVIT OF DISCREPANCY

I, RENY ANDAYA, of legal age, married, with residence at Brgy. Laurel, Tanauan City,
Batangas, after having been duly sworn to in accordance with law do hereby depose and state:

That my name listed therein is RENE ANDAYA and sometime I used the name RENY
ANDAYA;

That when I secured from the Office of the Tanauan Local Civil Registrar, I discovered
that the true spelling of my name is RENE ANDAYA;

That this affidavit is being executed to attest and prove my correct name as RENY
ANDAYA and not RENE ANDAYA and to certify further that the name RENY ANDAYA and RENE
ANDAYA refers to one and the same person.
