Personal History
Mohamed Mahmoud Hassan is a 47-year-old married clerk with 4 children, living in Fayed. He has no
special habits of medical importance.

C/O
Vomiting blood and blood in stool

HPI
Patient complained of attacks of hematemesis and black tarry stool, of acute onset and progressive course
that began one week ago, with no aggravating or relieving factors; the most recent attack began one day
ago after an EGD. The patient has HBV and liver cirrhosis (diagnosed in 2006), esophageal varices, and
peptic ulcer disease (diagnosed in 2008).

Past History
Patient had a hemorrhoidectomy in 1994 and a blood transfusion in January 2013. He does not have DM
or HTN.
