Is it agreed that The Tool pusher shall supervise the task on rig floor and Only Driller or AD

6 Is there a rescue plan been developed and reviewed
7 Is it confirmed that no SIMOPS or crane lifts to the Rig floor will be carried out
8 Has the drawworks been isolated (emergency brake applied)
9 Hand Signals -Have the RAISE, STOP and LOWER hand signal been agreed
10 Has the winch operator agreed the principle of 'NO SIGNAL NO MOVEMENT'
11 Is the Winch, harness, carabina/shackle and wire certified, ispected and in a good condition
12 Has the harness been adjusted properly, is the manrider wearing a helmet with chin strap
13 winch cable in good condition, and spooled correctly? (min 6 turns on drum,)
A fall arrestor shall also be used, test before use (connect to a separate point on harness if

possible)
15 Winch line connected directly to the harness and in good condition (No swivels/hooks)
16 Are the manual and automatic brakes and slack wire device operating correctly
Control lever operate correctly, is it clearly marked up/down, does it return to neutral

automatically?
18 Has the emergency stop facility on the man ridding winch been tested
19 Test the load limiter to max(150Kg) this also tests the wire before doing the man riding
20 Is it agreed that the man ridder will only be lowered at Rig floor
21 Has winch operator agreed he will not leave the winch while the person is manriding
22 Are the hand tools/ radio tied off to the harness/manrider securely and are his pockets empty
23 Has the man-riding register been completed
24 Is it agreed that the winch will be locked after use
25 All persons involved in operation went through this checklist prior commencement of the job
