1. GENERAL

1.1 Site working hours will be 06:00am to 20:00pm Monday to Saturday.

1.2 All piling plant operators and assistants are suitably qualified. A full‐time site foreman will
supervise the works.

The gang consists of:

‐ 1 no. Working foreman / supervisor.


‐ 1 no. Piling rig operator
‐ 1 no banksman.
‐ 1no. Pumpman
‐ 5 no. Assistants

1.3 All plant and equipment will have current test certificates in accordance with the relevant
statutory regulations.

1.4 Work permits and all approvals to be in place prior to commencement.

2. PLANT

The following major plant will be used:

(a) A purpose built track‐mounted piling rig with container tool box.

(b) Static concrete pump.

(c) Excavator.

(d) Site dumper or dumper lorries.

3. SITE ESTABLISHMENT AND SETTING OUT

3.1 Equipment will be unloaded and erected in accordance with standard procedures and the
manufacturer’s instructions. The augers will be lifted into position using the auxiliary winch,
and a pin will be placed to connect the male / female part of the augers together. As the
banksman will be working at height, he will be connected to a fall arrest lifeline, using a
harness.

3.2 Setting out will be undertaken using a steel marker pin, and maintained as necessary
throughout the contract. Prior to being left unattended setting out pins will be suitably marked
and driven flush with the platform level. Prior to work commencing, confirmation of the piling
platform level shall be obtained.

3.3 If the material forming the piling platform makes plan location of the auger difficult to
maintain, two offset marker pins are placed at right angles to the setting out pin by the piling
banksman.

The piling rig incorporates mast inclinometers to ensure the mast and auger is plumb prior to
commencement of drilling.

4. CONSTRUCTION METHOD

4.1 The piling rig will be erected in accordance with the manufacturer’s details, on the piling
platform.

4.2 Prior to commencement of the piling operation, an auger ‘end cap’ is fitted at the base of
the auger to seal the hollow stem against the ingress of ground water and soil, as the
continuous flight auger bores into the ground to the required depth. Little spoil is removed
during the boring phase.

4.3 On completion of boring the auger will be lifted by up to 100mm to release the ‘end cap’
prior to auger‐stem concrete precharging.

4.4 Concrete is pumped, under pressure and at a sufficient rate to ensure a continuous and
monolithic shaft, to the bottom of the auger through its hollow stem. When extracting the
auger, bore spoil on the auger is cleared using the auger cleaner on the piling rig assisted, if
necessary, by the attendant excavator. Cleaning of the auger by hand may be necessary when
working close to other operations or avoid excessive splashing.

4.5 The auger is withdrawn at a controlled rate of extraction, such that the volume of concrete
placed is slightly greater than the theoretical volume of the pile shaft. The extraction rate is
not necessarily constant and may be adjusted to allow for local variations in soil conditions.

4.6 Should a blockage occur the auger is extracted by rotating anti‐clockwise to leave as much
spoil supporting the hole as possible

4.7 Blockages are cleared with the use of cleaning balls passed through the pipe system
propelled with compressed air. The lines may be split as necessary at joints to aid this process.
Safety procedures must be followed at all times, especially ensuring the area is cleared of
nonoperational personnel, pipe ends are anchored / restrained mechanically and pressures are
released prior to clip loosening. In all instances, once the blockage is cleared the hole is
redrilled to a founding level slightly deeper than originally achieved.

4.8 Bore spoil arisings and concrete overbreak from boring and construction is removed from
the pile head by a full time attendant excavator.

4.9 The concrete pile is cast to ground/working platform level. Pile shaft concrete cannot be
left below the existing piling platform.

4.10 No lining tubes are required because at no time is there an open bore prior to concrete
injection.

4.11 The remaining arisings and concrete overbreak from the bore is removed by the full time
attendant excavator and the top of the concrete checked for contamination.

4.12 The prefabricated reinforcing cage is then, with the minimum delay, installed into the
concrete. The piling cage is brought from the place of storage to the side of the piling rig. The

prefabricated cage will be lifted in place using the auxiliary line on the piling rig, as well as the
attending machine. The cage will then be left attached to the auxiliary line and lowered under
its own weight. The attending machine may need to assist this operation by pushing the cage
into the workable grout to the required depth as detailed within the design documents. Should
the top of the cage remain above the cast grout level (platform level) the bars will be suitably
marked (with high visibility tape or paint), wherever possible reinforcement will be placed to
platform level.

4.13 The crawler mounted piling rig then moves to the next position in the agreed sequence.
This shall be sufficient distance as not to induce concrete flow from, and or, damage, to
adjacent piles.

5. CONCRETING

5.1 The concrete shall beC 30 (grade 30N/mm2)

5.2 During pile installation and prior to the extraction of the auger, the concrete is discharged
into the concrete pump and pumped under pressure through the delivery tube, to the base of
the auger stem.

5.3 To control and ensure continuous concreting, the monitoring shall include:

• The control of the supply of concrete (adequate concrete volume),

• The control of the concrete pressure,

• The control of the speed of extraction,

• The record of the speed of rotation of the auger.

Rig operator shall ensure adequate continuous monitoring of the above processes.

6. REINFORCEMENT

6.1 All reinforcement material shall be from an approved supplier.

6.2 A designated area will be assigned by the main contractor for steel fixing.

6.3 The steel fixing trestles will be positioned on firm ground (preferably an area of the piling
platform).

6.4 Steel will be lifted onto the fixing trestles, by the attending machine.

6.5 Steel fixers will fabricate one cage at a time using soft annealed tie wire, sufficiently tied to
allow handling and placement.

6.6 Fabricated cages will be lifted off the trestles with the aid of the attendant excavator and
stock piled within agreed storage areas.

7. RECORDS

7.1 Records of the pile lengths, reinforcement and concreting will be made daily and
forwarded to the Project Manager for signature and distributed as required.

7.2 Relevant computerized equipment to monitor shall be provided.

The boring and concreting parameters shall be continuously displayed, allowing the driller to
control in real time these parameters and engage the corrective actions where appropriate.
During the drilling phase, the penetration rate, the torque, the speed of rotation and the depth
of the auger will be displayed. Subsequently, during the concreting phase, a continuous record
of concrete pressure (measured at the head of hollow stem) and rate of pumping will be
shown. The display shall be presented in a manner that highlights under‐supply and over‐
supply of concrete, compared with the target value.

7.3. On completion of a pile, a computer printout can be generated for viewing, and can be
transmitted upon request to Contractor’s representative. The full data of pile construction
details shall be stored in a data logger module, which will be transmitted at regular intervals
(every day) to Contractor’s representative. These data shall be analyzed by Subcontractor, as a
further check on the integrity of each pile. Conclusions of these analyses shall be part of the
“As‐built pile foundation report” to be issued at the end of piling works in each concerned
area.

8 ANOMALIES

8.1 Anomaly (premature refusal) during the boring phase;

Causes;

i. Debris (dead woods, tires, scrap iron etc...).

ii. Existing building slabs and or reinforcement bars.

iii. Hard rock section.

Corrective actions;

Probe, depending on soil formation. For example in sandy lose soil where the water table is
high cannot be probed more than 2 meters deep, in this case a retaining wall is constructed for
easy probing. However when the soil is strong (e.g. laterite), probe and remove obstruction.

In the case of existing building slab or hard rock section, a rock breaker is needed then
excavator to excavate and remove then backfill and leave the area or point in question for a
while for compaction and strong enough to avoid backfilling during casting.

When the retreat of the auger is not right with the concrete pump (discontinuity between
auger and concrete)

Corrective actions;

i. Tight and pencil shaped mouth of cage

ii. Retrieve bent cage then turn and insert simultaneously

8.4 Anomaly (premature refusal of the cage + impossible retrieval) during the installation of
the reinforcing cage;

Causes;

Depending on the specification of the cage depth (12m, 6m...).

Corrective actions;

The protruding or remaining bars are cut off.

Depending on load and moment actions a supporting pile may be needed. This is decided by
the structural engineer.

9 AS‐BUILT PILE FOUNDATION REPORT

For each concerned AREA, Subcontractor shall prepare the “As Built Pile Foundation
Report”, aiming to present all the useful information in view of final acceptance of piling
works, including:

• Work procedure,

• Summary of quantities (number of piles, pile type, pile length),

• Interpreted results of preliminary tests (probe piles + pile load tests),

• Summary of non‐conformities and corrective actions,

• Incidents if any, interruptions, etc.

• Full data of pile construction details,

• Boring and concreting record sheets,

• Interpreted results of non‐destructive tests (control of pile integrity).

1. GENERAL

1.1 The objectives of installing preliminary test piles are to:

• Confirm that the construction of the piles as per the defined work procedure is
feasible, considering especially the site specific geotechnical conditions.

• Implement pile load tests on probe piles.

1.2 Probe piles will be non-working piles.

2. DRILLING

2.1 For each test pile, a “Probe Pile Test Sheet” shall be established during the boring phase,
including at least the information listed in the boring and concreting record sheet with the
following additional information:

• Geological description and characteristics of the various soil layers encountered,

Keeping in mind that these tests especially aim at confirming that the power of the tools and
the proposed equipment are adapted to the soil conditions.

3. CONCRETING

3.1 The concreting/grouting phase of the preliminary test piles aims at confirming that the
proposed concrete mix (and grouting pressure) is suitable to the execution of the piles. It also
enables to assess the concrete behaviour with time depending on the various additives, in
order to ensure satisfactory workability of the concrete.

3.2 For each test pile, a probe pile test sheet shall be established, including the record of
concreting or grouting parameters.

4. REINFORCEMENT

The phase of installation of the reinforcing cage (of the defined pile length) enables to validate
the proposed methodology (with possible use of vibrator if necessary).

1.- SCOPE

This test method statement covers procedures for testing vertical piles individually to
determine response of the pile to a static compressive load applied axially to the pile. This test
method is applicable to deep foundation units that function in a manner similar to piles
regardless of their method of installation, and specifically for precast and cast-in-situ piles.

The test will be performed according to ASTM D 1143-81 (Standard Test method for Piles
Under Static Compressive Load).

2.- PREPARATION OF PILES

Where feasible, the immediate area of the test pile is excavated to the proposed pile cut-off
elevation. The test pile shall be cut off or built up to he proper grade as necessary to permit
construction of the load-application apparatus, placement of the necessary testing, and
instrumentation.

If the head of the pile has been damaged during driving, the damaged portion shall be removed
prior to the test.

A sufficient number of anchor or anchoring device(s) will be installed so as to provide


adequate reactive capacity and a clear distance from the test pile over the test pile a test
beam(s) is placed of sufficient size and strength to avoid excessive deflection under load with
sufficient clearance between the bottom flange(s) of the test beam(s) and the top of the test pile
provide for the necessary bearing plates and hydraulic jack(s). The test will be executed no
before than 1 week after the anchors installation.

Test beam, is attached to the anchoring devices with connections designed to transfer the
applied loads to the anchors so as to slippage, rupture or excessive elongation of the
connections under maximum required test load.

3.- APPARATUS

All reference beams and wires shall be independent supported with supports firmly
embedded in the ground at clear distance of not less than 2.5 m from the test pile and as far as
practical from the anchor piles. Reference beams shall be sufficiently stiff to support the
instrumentation such that excessive variations in readings do no occur and should be cross-
connected to provide additional rigidity.

Dial gages shall have at least a 50 mm travel; gages shall have a precision of at least
0.25 mm. Smooth bearing surfaces (such as glass) perpendicular to the direction of gage-stem
travel shall be provided for the gage stems.

All dial gages, scales and reference points shall be clearly marked with a reference
number or letter to assist in recording data accurately. Provisions shall be made to protect the
measuring system, reference system, and instrumentation form adverse temperature variations
and from disturbance, All gages, scales or reference points attached to the test pile or pile cap
shall be mounted so as to prevent movement relative to the test pile or pile cap during the test.

A minimum of two dial gages shall be mounted on the reference beams


approximately equidistance from the centre of and on opposite sides of the test pile or pile cap
with stems parallel to the longitudinal axis of the pile(s).

The use of four dial gages mounted 90º apart is recommended to compensate for
lateral movement or rotation of the pile due to accidental eccentric loading.

Two pressure gages shall be mounted in the pressure system (one on the hydraulic
jack, and one on the hydraulic pump).

4.- LOADING PROCEDURES

Standard Loading Procedure

Unless failure occurs the pile is loaded 150 to 200% of the anticipated pile design load
for applying the load in increments of 25% of the individual pile design load. Maintain each
load increment until the rate of settlement is not greater than 0.25 mm/h., but no longer than 2
h. Provided that the test pile has not failed, remove the total test load any time after 6 h. if the
butt settlement over a one-hour period is no greater than0.25 mm.; otherwise allow the total
load to remain on the pile or pile group for 12 h. After the required holding time, remove the
test load in decrements of 25% of the total test load with ½ h between decrements.

Readings of time, load, and movement are recorded before and after the application of each
load increment or the removal of each load decrement. During loading, provided that the test
pile has not failed, additional readings are performed and record at intervals not exceeding 10
min during the first ½ h nor 20 min thereafter for each load increment. After the total load has
been applied, provided that the test pile or pile group has not failed, readings are performed at
intervals not exceeding 20 min during the first 2 h, not exceeding 1 h for the next 10 h, and not
exceeding 2 h for the test 12 h. If pile failure occurs, take readings immediately before
removing the first load decrement. During unloading, take readings and record at intervals not
exceeding 20 min take a final rebound reading 12 h after all load has been removed.

Only authorized personnel shall be permitted within the immediate test area.

1.- SCOPE

This test method statement covers procedures for testing piles either individually or in groups
to determine the load-defection relationship when subjected to lateral loading. It’s applicable
to all deep foundation units regardless of their size or method of installation, but specially
indicated to precast and cast-in-situ piles.

The actual lateral load capacity of the pile-soil system can best be determined by lateral
testing. Such testing measures the response of the pile soil system to lateral loads and may
provide data for research and development, engineering design, quality control, and acceptance
or rejection under specifications.

3.- APPARATUS FOR APPLYING LOADS

The apparatus for applying lateral loads to the test pile(s) shall be constructed so that the
resultant loads are applied horizontally and in line with the central vertical axis of the pile or
pile group so as to minimize eccentric loading an avoid a vertical load component.

Before applying the test load, any annular space around the upper portion of the test pile(s)
should be filled with sand or other suitable material.

Lateral test loads shall be applied at approximately pile cut-off elevation.

A steel test plate of sufficient stiffness to prevent it from bending under the involved loads, but
not less than 2 in. (50 mm.) thick, shall be set vertically against the side of the pile, pile cap, or
steel frame at the point(s) of load application and perpendicular to the line(s) of load
application. The test plate shall be of sufficient size to accommodate the hydraulic cylinders
but shall have a horizontal side dimension not less than one half the diameter or side
dimension of the test pile(s) nor greater than the diameter or side dimension of the test pile(s).
For tests on single piles other than square piles, the head of the pile shall be capped so as to
provide a plane vertical bearing surface for the test plate or, the test plate shall be capped so as

to provide a plane vertical bearing surface for the test plate or, the test plate shall be set in
high-strength grout or adequately welded to the side of the pile using suitable filler material to
provide full bearing against the projected area of the pile.

Testing equipment: Lateral loads shall be applied using one or more hydraulic cylinders. If
two or more hydraulic cylinders are to be used to apply the test load, they shall be of the same
piston diameter, connected to a common manifold and pressure gage, and operated by a single
hydraulic pump.

Load Applied by Hydraulic jack(s) Acting Against a Reaction System:

Apply the test loads to the pile or pile group using one or more hydraulic cylinders and a
suitable reaction system. The reaction system may be any convenient distance from the test
pile or pile group and shall provide a resistance greater than the anticipated maximum lateral
test load. Set the hydraulic cylinder against the test plate at the point of load application in a
horizontal position and on the line of load application.

Reaction piles. Install the necessary number of reaction piles vertically or on a batter (or a
combination of vertical and batter) so as to provide the necessary reactive capacity for the
maximum anticipated lateral test loads.

Deadman. Where soil or site conditions are suitable, install a deadman consisting of cribbing,

Load Applied by Hydraulic Jack(s) Acting Between Two Test Piles or Test Pile Groups. Test
the lateral capacity of two single piles or two similar pile groups simultaneously by applying
either a compressive or tensile force between the pile or pile groups with a hydraulic jack(s).
Test piles or test groups may be any convenient distance apart. If necessary, insert a steel
strut(s) between the hydraulic cylinder(s) and one of the test piles or groups.

3.- APPARATUS FOR MEASURING MOVEMENTS

Set all reference beams and wires level and support them independently with supports firmly
embedded in the ground and at a clear distance of not less than 7 ft (2 m) form the test pile(s)
or group. References beams shall be of sufficient axial a lateral rigidity to provide stable
reference points for pile deflection measurements.

Dial gage stems shall have at least (75 mm.) travel and sufficient gage blocks shall be
provided.

Clearly mark al dial gages, scales, and reference points with a reference number. Protect the
instrumentation measuring system and reference system from adverse temperature variations
and from accidental disturbance. Mount all gages, scales, or reference points so as to prevent
movement relative to their support system during the test.

Procedures for Measuring Movements

Take required readings at each properly identified gage, scale, or reference point as nearly
simultaneously as practicable.

Standard Measuring Procedures. Take and record readings of time, load, and movement
immediately before and after the application of each load increment and the removal of each
load decrement. Take and record additional readings at 5 min intervals between load
increments a load decrements. While the total test load is applied, take and record readings 15
min. and 30 min. after the total load has been removed. If pile failure occurs, take the reading
immediately before removing the first load decrement.

1. INTRODUCTION

Pile Integrity Testing is a reliable method to quickly evaluate the quality of installed pile shafts.
The method provides fast, reliable and inexpensive tool to assess pile integrity. Generally, the
Pulse Echo Method (PEM) that uses only velocity measurement is widely used and
recommended. It provides velocity data on structural elements (i.e. structural columns, driven
cast‐in‐situ piles, bored piles, concrete filled steel pipe piles, timber piles, etc.). This data
assists evaluation of pile integrity and detect potentially dangerous defects such as major
cracks, necking, soil inclusions or voids and, in some situations, can determine unknown length
of pile that support existing bridges or towers. However pile capacity cannot be determined by
this method.

The test procedure conforms to ASTM D5882‐2007.

2. SITE PREPARATION & DATA COLLECTION

a) For cast‐in‐place concrete piles or concrete filled pipe piles, the integrity testing is
performed 7 days after casting or after concrete strength achieves at least 75% of its design
strength, whichever occurs earlier.

b) The pile shall be tested at sound concrete level after removal of muck and debris from the
top. The projecting rebar, if any needs to be bent outside for easy access within core region.
No high strength mortar or cement paste shall be applied on the pile top to make it smooth
and even. However, the pile head with sound concrete should be fairly leveled, clean, dry and
dust free, etc.

c) The pile integrity testing is conducted by attaching an accelerometer onto the pile top
generally near the center of the pile (not near its edge) with the help of bonding material like
candle wax, putty, etc.

d) After attachment, the pile head is impacted with a hand held device (nylon tipped hammer).
This generates low stress wave into the pile, thereby resulting strains of very low magnitude
and hence the method is called Low ‐Strain Integrity Test.

e) The accelerations generated by the impact are measured by the accelerometer attached on
the pile top and are converted to velocity form for display onto the collector screen. All
reflections either from pile toe / pile discontinuities, cross‐sectional changes, soil resistance
changes, etc., are graphically displayed.

f) The test involves collection of several blows during the stage of testing. All such similar
blows are averaged before display. This averaging technique tends to cancel random signals in
any particular blow while amplifying the effects of relevant repetitive response.

g) The signals are also exponentially amplified. Low strain signals generated due to hammer
impact are often damped by skin friction. For long piles with high skin friction, reflections from
pile toe may be small. By amplifying the records exponentially with time, this will enhance the
identification of relevant reflections that have low energy.

3. ANALYSIS OF DATA

A stress wave (sound wave) is introduced into the pile by means of a hammer blow on the pile
head (Fig.3).

This stress wave travels at the speed of sound (c) to the pile toe and reflects back to the pile
head.

The response of the pile head, as a result of the hammer blow and reflections, is measured
with an acceleration transducer. The acceleration is integrated and presented as a velocity
signal (v). For each pile at least 3 hammer blows are applied to the pile head and the results
are presented as 3 traces in a diagram (Fig 4). To proof the quality of testing the 3 traces
should be similar.

The time (T) between the start of the hammer blow and the time of arrival of the reflection
from the pile toe is measured. The pile length (L) is calculated with:

L = c.T/2

when the stress wave velocity ( c ) is known.

To present the measuring results the time axis (t) is scaled to a length (depth) (l) axis with
L= c.t/2. Due to shaft friction the toe reflection might be of small magnitude. To make the
reflection visible, the measured signal is amplified. To remove noise from the signals a filter
value can be applied.

Pile integrity testing analysis is based on the one dimensional stress wave theory. Reflections
generated by impedance changes (discontinuities) travel to the pile top and are recorded and
analyzed.

The impedance Z is defined as

1. - DESCRIPTION OF WORK

CFA piles are formed by drilling to the required depth using a hollow stem continuous flight auger.
After reaching the design depth, a high slump concrete is then pumped through the hollow stem.
While the concrete is being pumped, the auger is withdrawn at a controlled rate, removing the soil
and forming a shaft of fluid concrete extending to ground level. The reinforcing cage is then inserted
into the fluid concrete, with possible assistance of cage vibrators (pending on cage length).

The boring phase for this type of pile is performed continuously with a hollow auger, screwed in the
soil in order to reach the medium dense or dense sand layer and to penetrate it on a minimum
height of 3 diameters (2.0m for a 0.6mØ pile). The soil which is not extracted during the boring phase
and fills the screw of the auger retains the wall of the boring hole. In order to ensure this stability, it
is necessary to adjust the speed of rotation of the auger to the type of soil in order to limit the
extracted volumes. A sealing device will be set in place on the axis of the auger to prevent soil and
water inflow during the boring phase.
The boring shall be performed as quickly as possible and with the minimum number of rotations, in
order to minimize the effects on the surrounding soil. When the target depth of boring is reached,
the auger is retrieved without unscrewing, in order to maintain the stability of the boring hole with
the column of soil material between the blades of the auger and to replace the soil material with
concrete during the raising.

When the boring has reached the target depth, the concrete is introduced into the central hollow
tube (closed during the boring phase), in order to fill the pile progressively during the raising of the
auger. The boring performed with the auger shall be completely full of concrete in order to form a
monolithic, continuous and sound shaft on the whole section of the pile and the whole required
height.
During the concreting phase, at the tip of the auger, the concrete is maintained at a pressure higher
than the external pressure, so that the void let by the raising auger is filled simultaneously by the
concrete. The concreting phase is carried out until the contaminated concrete in the upper part of
the column of concrete have exceeded the cut‐off level.

The CFA piles are reinforced on their whole height with reinforcing cages, which are inserted in the
boring after the achievement of the concreting, with possible assistance of cage vibrators (if
necessary pending on the pile length). Bars overlapping should be considered at the bottom of the

cage. The cages are equipped with large spacers of suitable shape, disposed on the whole height in
order to ensure the alignment of the cage with the axis of the pile during the lowering phase.

2. – PREVENTIVE ACTION

The first concept to consider in the study of risk and preventive measures is to integrate safety
measures into the process of execution of the job. Through proper behavioral conduct following
basic preventive rules and the use of individual and collective protective equipment by the staff,
bearing in mind that the means of collective protection are independent of the individual its
protecting, while the personal protection of the worker is dependent in the individual but
independent of the source of danger. This implies that collective means of protection should take
precedence over personal protection, leaving them only for situations that cannot be resolved by the
collective measures.

3. - RISK IDENTIFICATION AND PREVENTIVE MEASURES DESIGNED FOR


THE PRODUCTION OF PILES IN EACH OF ITS PHASES OF EXECUTION

3.1. Safety checks on work site.


Prior to the execution piles and the maneuvering of a drill rig on site it is necessary to have a
substantially horizontal work surface free of obstacles be it aerial or underground and wide enough
for movement of the machinery, material handling and adequate access. It is also necessary to
provide drainage for this surface to avoid pending, but remain firm and sturdy enough for the
machinery to be used. Check that there are no diverts, underground utilities that may be affected by
the excavations or any instability that may arise from them, also check that there are no obstructions
to the mast in the air like poles or cables. In the case of drilling close to a point of risk or a service
line, the position of such obstacles must be clearly and accurately marked /signaled.

You should also recognize the stability conditions of immediate structures to site, traffic flow of
vehicles and pedestrians in the immediate surrounding environment and other circumstances that
may arise, which must be taken into account to avoid the risk of accidents, to both the workers and
people outside the site.

The risks and preventive measures outlined below, serve both day and night work, with the
difference that for night work or low light, will have to use of lighting towers or lighting devices, both
the work area and in the walkways to achieving adequate lighting levels

3.2. - General risks associated with the activity

3.3. - Safety rules or preventive measures to be adopted

a) General safety rules


− Use machines with CE certificate conformity or certificate to the RD 1215/97.
− All persons handling machines shall be authorized by the company, will have the training and
specific information for the operation of the machine, also have a copy of the instruction
manual of the machine to be handled. Unauthorized persons will be prohibited from handling
machinery or equipment.
− All operators involved in this activity and maintenance people should know and fully
understand the necessary precautions before starting operations or maintenance work on
machines. "SAFETY FIRST" is always the first consideration for all staff, while working on a site
in normal or extraordinary conditions.
− Only qualified operators and maintenance personnel are allowed to work on machinery.
− If you have any questions about the procedure or handling machinery, immediate supervisor
shall be consulted for assistance and / or refer to the instruction manuals of equipment and
operating procedures.

− Always use protective equipment necessary for the work to be done (helmet, boots, gloves,
hearing protection, safety glasses, masks, etc.) Should avoid wearing loose clothing or in poor
condition.
− The hands, arms, legs and clothes will stay away from moving parts of machinery, including
(caterpillars, chain and chain pulleys progress, drilling pipes and hydrants).
− In case of having to handle harmful substances one should read and take into account every
instruction and information on the technical and safety data sheet.
− If safety or protective measures are dismounted from the machine for maintenance or any
other works, they should be reset before resuming work.
− Equipment will be used exclusively for that for which they were designed.
− Direction, safety and warning signs must be easily identified and kept clean, legible and in good
working condition.
− Should periodically inspect the surroundings of the machinery for potential risk situations.
− In case of having to hit rods, pipe or metal objects in order to loosen during unscrewing, you
must use safety glasses for risk flying projections.
− If machine parts are eroded or broken, or notice leakage in hydraulic lines or air pipes, work
will stop and there will be a general review of the entire machine.
− Always use the correct size cables with staples needed for the winch capacity and ensure that
all have secure fastening hooks in perfect condition.
− Observe and follow electricity and gas companies caution signs and rules when working near
power lines and gas pipes or areas where electricity cables buried.
− All workers before daily use, should review their personal protective equipment, and should
ask for their superiors for a replacement if any of their equipment is damaged.
− During night work or those where natural lighting is insufficient, proper lighting must be
provided for the proper execution of the work.
− The site is periodically cleaned of residual material.

b) Installation of equipment and checks to be made before commencement of works


− All equipments should have all the security measures in place such as:
¾ Acoustic signaling translation movements.
¾ Ensure that all lifting hooks have safety latch.
¾ Emergency Stop Device.
¾ Ensure that all cable ties come with adequate clips and are properly positioned in
construction of slings (preferably using cables and slings with leaded ties).
¾ Ensure that all equipments and machines have regulatory safety signs
.
− The operation of the machinery is limited to only trained and authorized personnel; it is
prohibited for anyone who does not have the required knowledge and authorization to
operate any machine.

− Make sure you understand the service procedures before operating the machine. Do not
attempt a job that is not understood. Do not use tools without the appropriate knowledge.
− Regardless of the rules in this document, it should not be forgotten that each machine has a
specific instruction manual from the manufacturer. This manual should be read completely and
understood before attempting to operate the controls or carry out any maintenance, assembly‐
disassembly.
− Carefully read all safety messages included in the instruction manual and machine safety labels
found in the machinery. Keep safety labels in good condition. Replace those lost or damaged.
− The operator must know the maximum loads for a given boom angle, which will be marked by
an inclinometer, he also need to know the weight of loads handled. Cargo transportation must
be done with movement of the mast, remaining stationary while moving loads. These loads are
maneuvered using resilient long ropes. Check that the appropriate slings are used for the size
and weight of the loads to be moved and that they are in good working condition.
− Avoid unexpected starting of the machine and make sure there is no possibility of risk when
started without taking proper precautions. Start the machine only from the operator’s panel
and give proper indications to other workers on site before any maneuver.
− Before starting the workday, it is desirable to carry out a visual revision for safe working
conditions. These reviews are important because they can detect abnormalities of the
elements of machines that can prevent accidents, both in people and in the lifespan of the
system.
− The machinery and auxiliary equipment are prepared to work in safe conditions, however,
unforeseen conditions, such as: pipe rupture, falling parts, wire breakage, etc. pose a safety
risk. For the purposes of safety of operators and helpers, they should always work with helmet,
boots, gloves and other personal protective equipment.
− When you need to access the mast to perform any maintenance or repair will be done with fall
arrester.
− within the system there are moving parts, such as: cables, winches, hooks, sliding table, etc. so
it is necessary to keep your hands, arms, legs and clothes (in good condition) outside the scope
of these parts, in order to avoid getting caught. It is necessary to note that most of the moving
parts are protected by fixed guards (plates and shells), which must not be removed under any
circumstances with the machine running.
− The area close to the work area of the machine need to be clean of elements that may cause
(falls, cuts, etc.) As well as walkways and / or access should be free of grease and oil.
− The person responsible for the whole system to work is the main operator, who is in charge of
control cabin, because from this position he controls all machine operations and the risks
involved.

c) Drilling
− During drilling, hoses shall be installed and arranged in an orderly manner so as not to affect
human and machine traffic on site. You should try to have the work area as clean as possible.
− While the machine is performing the drilling operation it is prohibit for any workers to remain
within the range of the work axis of the cutting tool.

− During excavation with the Piling rig, workers should be at a safe distance to avoid being
affected by a possible projection of particles or material.
− There should be a perfect coordination between the operator and assistants, with constant
visual contact.
− There should be good coordination at the time of evacuation of earth debris.
− After the drilling, the bore should be cleaned and earth debris evacuated thereof.
− While loading the excavated soil, there will be a person controlling the proximity of the
backhoe to the bore and other machines.
− The discharge of excavated by the Piling rig will be done without applying excessive rotational
force, thereby avoiding projections.
− Workers are prohibited from standing next to the Bored hole.
− The loading of trucks will be in an area outside the work range of the Piling rig, the driver
should be out of his cabin and with personal protective equipment in place during loading.
− The areas to be piled should be clearly marked and taped to prevent workers from passing
though the work Drill work zone.
− Drilling operations and earth removal may not run simultaneously. Earth removal and loading
on the truck will run only on completion of pile and concreting.

d) Introduction of Steel cage.


− The reception and collection of armor will be made in specified area.
− Hooks are placed in the Steel cage for handling and suspension, this hooks must be firmly
attached to the rest of the armor.
− Before hoisting the steel cage, the supervisor in charge will carry out a visual inspection of the
same, in order to detect any loose or incorrectly bound objects that may cause injury if
projected.
− During hoisting maneuver of the reinforcement, it is strictly prohibited for anyone other than
the team in charge of this maneuver to be present in the area work.
− Only the necessary personnel will help in guiding the steel cage into the bored hole, staff not
required for this operation must maintain the required safe distance from the work area.
− When the cage is suspended, it will be guided by ropes attached to the free end, never with
bare hands.
− The discharge of the steel cage will done by suspending two distant points using a rigid beam
that will depend on the crane hook.
− It is forbidden to hoist the reinforcement to its upright position by jerking.
− It is prohibited for workers stay within the working axis of the machine while hoisting the
reinforcement, until it is fully upright and has stop moving.
− The placement of the steel cage into the pile mouth will be done with ropes. It should not be
handled.

e) Concreting
− While concreting is compulsory to wear safety glasses to protect against particles.
− They must wait for the truck mixer to stop completely before deploying the chute.
− Strictly prohibit all personnel from standing behind vehicles or machines that are working.
− Prohibited to stay in the concreting area until the truck is positioned and stopped and ready for
pouring.
− The approach maneuvers of the concrete mixer truck to a disposal site, will be assisted by
visual signs from an assigned personnel.
− Whenever the pile exceeds the ground surface, and requires the pile head to be opened, his
steel cage is suitably protected.

Safety Measures

General Recommendations
− Use CE marked drilling rigs primarily or those adapted to RD 1215/1997.
− It is recommended that the Drill rig is equipped with rotary type flashing light or flash.
− Must have reverse beeper.
− Before commencement of the work, check that the operator is authorized, has the required
training and information specified by PRL according to RD 1215/1997, of July 18, Article 5, and
that he has read the instruction manual.
− Piling operations must be manned by a competent supervisor.
− Before starting work, check the cables, make sure that all devices are working properly and the
Drill rig is in perfect condition: brakes, chains, etc.
− It is forbidden to use a mobile phone, except there is a hands free kit available.
− The seat and controllers must be set to the proper working position of the driver.
− Ensure maximum visibility in the piling rig by cleaning mirrors, windshields and mirrors.
− Make sure the cabin is clean, free from oil, grease or mud with no uncontrolled objects in the
area of controls.
− The operator has to clean his shoes before using the stairs to the cabin.
− The operator should climb in and out of the cabin using steps designed by the manufacturer,
facing the machine and holding the access bars with both hands.
− Check that all safety sign are intact and placed in a visible places.
− Check the fire extinguisher is in its place on the Rig, periodically verify.
− Ensure that the maximum height of the piling rig is adequate to avoid interference with other
elements such as bridges, power lines or similar aerial obstructions.
− Maintain clean all access routes, handrails and steps.

Specific Recommendations
− Operate machine only from the operator's seat.
− Do not allow the presence of workers or others in the operating axis of the machine.
− Do not allow outsiders to be transported by any machinery.
− No climbing up or down the machine when it is in motion.
− While operating, always use a restraint system (cabin seat belt or similar).
− To restart an activity after significant rain fall, you must take into consideration that the ground
conditions may have changed. Check the track and brake systems.
− When operating in areas near electric cables, you have to check the voltage thereof to identify
the minimum working distance.

be supported on the floor.
− The waste generated as a result of a breakdown or it maintenance must be segregated in
containers.
− In transport operations, check if the length, tare and blocking and bracing system are
adequate. Also, it must be ensured that access ramps can support the weight of the Piling rig
and once loaded the key must be removed from the contact.
− Park the Piling rig in appropriate areas. In a Flat, solid ground, safe from crashes, landslides or
flooding. (At least 2 m from the edges of any slope). We must put the brakes, remove the keys
from the ignition, close the battery switch and close the cockpit and the engine compartment.

− Before starting work, ensure that all devices on the self-propelled pump are working properly
and are in perfect condition: cables, brakes, tires, etc.
− To use a mobile phone while driving, you must have a hands‐free system. In vehicles with
sensitive electronic systems, their use is prohibited.

− The pump must only be installed on a compact stable zone before working with loads.

3. Controls Plan
Evomec Global Services Limited shall establish an earned value management system to integrate all
the major components of the project and baselined. These major components of the earned value
management system shall include scope, schedule and cost. The cost and schedule performance shall
be measured against the project baseline to help track the progress of the project.
Based on the Project Scope and available resources, the work activities in the WBS shall be scheduled to
establish the Schedule Baseline. The allocation of resources across the schedule for each element of the
project’s scope shall be Evomec’s tool for creating the project’s integrated baseline. The scope, cost,
and schedule shall be fully integrated in our plan to be able to perform Earned Value Management (EVM).
As the chart shows below, each task shall be assigned an associated schedule and time phased cost.
Evomec planning package shall have a firm budget, estimated start and complete dates, and Statement of
Work.
As work becomes more clearly defined, Planning Packages are then converted into Work Packages,
with the following constraints. These constraints shall ensure the initial budget is used appropriately:
All planning packages are converted into work packages as requirements are defined, and at a minimum,
are scheduled to start at least one month beyond the current reporting period.

the Undistributed Budget. PMB does not include Management Reserve.
Distributed Budgets include budgets assigned to control accounts during baseline establishment as well as
work packages and planning packages while undistributed budget (UB) applies to contractually authorized
efforts not yet allocated to WBS elements. The UB shall consist of a budget for authorized changes for
which there has not been adequate time to plan the change at the control account level. Undistributed
budget shall also be used as an element of the Performance Measurement Baseline. Evomec shall put every
effort in place to distribute budgets in a timely manner and to minimize undistributed budgets.

4. Procurement Plan

Evomec project procurement plan facility shall allow planning and tracking of “plan records”
against any definable milestones associated with the procurement cycle. The term “plan record”
will normally be synonymous with a requisition but the system allows for more than one plan
record associated with a requisition for situations when different planning / tracking is needed,
when requisition items are assigned to more than one purchase order. A key milestone in the
plan is the one that is defined as the “end” that will be compared with a defined Required On Site
date for evaluation of the number of days “float”.

a) KPIs and Measurable Targets

Besides the ability to ensure that materials are delivered on schedule, the system is designed to
set measurable targets. This shall enable Evomec meet the various key performance indicators.
The establishment of milestones that may be anywhere along the entire supply chain as arranged
on the project schedule makes Evomec procurement plan a particularly effective management tool
for this project.

b) Flexible tracking level

Different requisitions are able to follow appropriate planning tracks based on the selected
Milestone Set. This permits configuration such that all requisitions can be linked to a procurement
plan to use the minimum Milestone representing the on-site delivery date so that realistic targets
durations up to the Required On Site date will be set. When appropriate, some requisitions can be
tracked through extra milestones (e.g. requisitions for high value engineered equipment).

5. Subcontracting Plan

o Selection of Subcontractors

Evomec shall record details of approved subcontractors on Subcontractor Form - New Supplier or
Subcontractor. Evomec Estimating Department will ensure that all subcontractors invited to
tender on projects are issued with HSEFM007 EGSL Form – Contractor Tender Invitation. The EGSL
Contract Department, in consultation with the Project Manager/ Contract Administrator will, on
receipt of quotations from subcontractors, review the quotation and select a subcontractor.

o Issuing of Contracts

The Project Manager will ensure that all subcontractors are issued with a Sub Contract Order prior
to commencing work on Evomec projects. It is the responsibility of the subcontractor to ensure
that the Sub Contract Order is read, understood, agreed to, signed and returned to the Project
Manager in a timely manner.

o Subcontractor HSE Requirements

Evomec shall ensure that all subcontractors undertaking work on projects under the control of
Evomec are made aware of their HSE requirements prior to commencing work on site. The Project
manager will be responsible for issuing a Sub Contract Order to all subcontractors undertaking work
on projects under the control of Evomec. The Sub Contract Order includes an attachment outlining
subcontractor HSE requirements prior to commencing work on site.

Evomec shall ensure that Subcontractors undertaking work on its projects are involved in HSE
inspections and audits. The Site Manager/Supervisor will involve subcontractors or their
representatives in daily HSE inspections. The subcontractor will sign the JSA Form – HSE Daily
Inspection to verify involvement. The Evomec HSE Officer will involve subcontractors or their
representatives in the Evomec auditing process through task observation, formal and informal
discussions Subcontractors will, where applicable be involved in other inspections such as:

• mobile plant and equipment pre starts

• lifting gear inspections


• fall arrester inspections
o Monitor and Review

Subcontractor HSE performance will be monitored and reviewed throughout the life of the project.

6. Risk Management and Contingency Planning.

Overall responsibility for risk management within OTUMARA Piling Project shall be the
responsibility of the Project Manager, with responsibility for implementation delegated to the
Site Manager/ Supervisor.

The Project Manager has overall responsibility for managing project risk. Project team members
shall be assigned specific areas of responsibility for reporting to the project manager.

Throughout all phases of the project, a specific topic of discussion will be risk identification. The
intent is to instruct the project team in the need for risk awareness, identification, documentation
and communication.

The project manager is responsible for logging the risk to the Risk Register. Notification of a new
risk shall include the following Risk Register elements:

• Description of the risk factor or event, e.g. conflicting project or operational initiatives
that place demands on project resources, unexpected study outcomes, delays, etc.
• Probability that the event will occur. For example, a 50% chance that the vendor will not
have materials procured that meets the project specification available.
• Schedule Impact. The number of hours, days, week, or months that a risk factor could
impact the schedule.
• Scope Impact. The impact the risk will have on the envisioned accomplishments of the
project. Delayed material delivery may result in a reduction in the number of piles that
can be completed within the contract period of performance.
• Quality Impact. A risk event may result in a reduction in the quality of work or products
that are developed.
• Cost Impact. The impact the risk event, if it occurs is likely to have on the project budget.
The Audit Committee reports on the arrangements for risk management to the Board of
Directors.

7. Document Control Plan

o Overview

Evomec Project Manager/ Construction Manager (PM/CM) shall provide and operate a
document control system to support the OTUMARA Piling Project that is in compliance
with the project specifications for Document Control. The document control system shall:

 Provide for a complete set of electronic project files of all projects.


 Establish lines of communication among all Project participants.
 Provide for processing all project documents for review and comment.
 Provide a process to identify project documents by the specific project number.

 Provide for a means of extraction, conveyance and tracking of documents in electronic form
within Evomec database.
 Maintain proper documentation to support the baseline management activities in the Project
Management Plan and rigorously reflect the project baseline.

Whenever a hard copy project documents, especially those bearing original signatures or of a
sensitive or contractual nature, they will be scanned to create an electronic record by the
document controller and then transferred to Evomec project document data base. All
controlled documents shall be reviewed at least annually and updated as needed, as a result
of continuous process improvement efforts by the project management team.

o Scope

The scope of Evomec Document Control Plan shall be to:

 Standardize procedures for document control;


 Assure that documents are safely secured, maintained and readily available for use by persons
with access approval;
 Index documents received or collected for systematic filing;
 Preserve records of quality including: design documents, construction documents, contracts
and agreements, basis for design, construction, procurement and Owner operations and
maintenance records;
 Support the project processes of Change Control, Configuration Management and Quality
Assurance; and,
 Assure that all Project participants are consistently informed of the current policies,
procedures and basis for design, construction and procurement, including facility and systems
configurations and interface.

All documents requiring Client approval must go through a compliance review and technical
review by the Project Manager before going to the Client for review. The compliance review
checks whether or not the document complies with the template and matches the deliverable
description provided in the Project Plan. The technical review involves the Project Manager
reviewing the content for quality.

Once necessary changes arising from the compliance and technical review are made, the
Project document controller then transmits the document to the Client for review and
approval.

8. HSSE Plan

Evomec Global Services Limited has a well-defined Safety Management System (SMS) from
which it develops Project Specific HSE Plans, such as this one. To aid in this process, Evomec
both follows and utilizes the following guidelines in their development, and to stay current
with the various Regulatory Guidelines to which they must follow:

on board all operational equipment.

Evomec utilizes the philosophy that all company and sub-contract employees are responsible in
performing occasional Self Audits and are equally responsible in auditing their particular work area.
Where applicable Audit Action Items will be documented and specific Target Dates applied for
completion. These actions will be addressed during the Weekly Safety Meetings and/or Daily Toolbox
Meetings, where necessary

iii. Hazard Communication (HAZCOM) – OSHA - §1910.1200

EVOMEC GLOBAL Operations will utilize a Hazard Communication Program to ensure that personnel on the
Project understand the hazards and safe work practices associated with piling operations and associated
chemicals used. A comprehensive inventory of Material Safety Data sheets (MSDS) will be kept on site and
each departmental office shall maintain their own MSDS records, which
will be amended, as and when necessary. All hazardous materials that are brought to the site must
be accompanied with a MSDS

iv. Safety Performance Recognition/Incentive Program

There shall be a Notice Board in the project common area. All General Communications applicable to the
overall project crew are posted on the notice board. Likewise, the Safety Meeting &
Toolbox Meeting media is also used to convey Safety Performance. v.

Project Risk Assessment & Management

A Project Specific Risk Assessment shall be generated by Evomec HSSE Team, this shall be taken
from a Generic List of potential hazards that the company maintains on file. All potential Risks are
evaluated and priorities given in order to identify those of higher risk.

Control measures (mitigations) are identified as either existing, or when necessary new ones are
implemented to move the Risk or at least minimize through control measures and procedures. All control
methods and procedures are monitored to ensure their continued effectiveness. If they are not
satisfactory, these hazards must be reassessed and/or controls and procedures revised. All revisions will
be recorded in the Generic Hazard List.

9. Quality Plan
o Quality System Description

Evomec quality management system is planned, documented, and implemented to provide management
and client’s assurance that project activities are planned and performed in accordance with approved
processes and requirements. The information provided in this quality plan identifies and defines when
and how controls are to be applied to our technical or project processes and operations or makes
reference to where in this plan quality assurance and control activities are planned, implemented, and
assessed to ensure that the project executions are credible and reliable. The rigor with which controls
are applied is determined using a graded approach. Graded approach is defined as the process of basing
the level of application of managerial controls applied to an item or work according to the intended use
of the results and the degree of confidence needed in the quality of the results. Evomec’s quality
management system applies to projects needs assessment, project planning and reviews, contractor
selections, project management, report approval, and project assessment and improvement.

o Management and Organisation

Evomec’s management has established the mission, vision, and goals of the organization as described in our
comprehensive quality plan. Our quality policy defines the objectives and goals of the quality program
which are intended to ensure that processes produce the type and quality of results needed and expected.
To this purpose, managers and staff as described in our quality plan have been assigned functional
responsibilities, authority, accountability, and resources necessary for managing and implementing
Evomec’s quality process as described in this Quality Management Plan (QMP).

Management has identified its contractors and the vendors who provide the services required to satisfy
their needs and have communicated the requirements of this plan to its staff and contractors. The
quality plan is regularly assessed to determine its implementation and effectiveness.

o Training

The qualifications and experience of our personnel are evaluated by the management staff. Further
training, including the need for specific qualifications and certifications, shall be provided as required to
ensure that personnel are trained and qualified based on project-specific requirements prior to the start
of work. The QA Lead shall direct the creation and maintenance of training records. Training and
qualifications duties and responsibilities for performing training and maintaining records of training are
described in full in Evomec Quality Management Plan.

o Quality System Implementation

Evomec shall conduct all construction activities in accordance with the requirements of Quality
Management Specification and all plans or policies developed by the Client to satisfy the requirements
of this Specification.

o Procurement

The procurement of project materials and other deliverables are planned and controlled in accordance
with the Evomec’s procurement Plan. Once proposals have been reviewed and approved, project
funding agreements are negotiated with the contractor. These agreements contain information that
clearly describes the service required and the quality requirements for which the contractor is
responsible, and how conformance with requirements will be maintained
via periodic performance reports. The funding agreements are reviewed for accuracy and completeness
by the Contracts Administrator prior to release. Changes to the agreements receive the same level of
review and approval.

o Quality System Documentation

The Document Controller is responsible for document and record control. Processes have been
established for identifying, preparing, reviewing, approving, revising, collecting, indexing, filing,
storing, maintaining, retrieving, distributing, and disposing of quality documents and records.
Documents and records that describe control processes and outputs are reviewed and approved prior to
use, distributed to the point of use, and revised as required. Personnel are retrained on the revised
version of the procedure.

o Implementation of Work Processes

Work is performed in accordance with approved planning and technical documents, including Project
Implementation Plans (PIP) as required. Procedures have been developed in a readable format with
sufficient detail to provide adequate guidance. Procedures have been implemented for routine,
standard processes and critical or special operations, as needed, and their implementation and
effectiveness is monitored to ensure their continued relevance.

o Quality Assessments

Once proposals have been reviewed, awardees are provided a contract that includes responsibilities for
quality performance. Periodic project progress reports are required to be submitted in a pre-approved
format with specific data fields. These reports are reviewed by the Project Manager and Contract
Manager to identify any issues that may adversely impact project progress.

Once a draft agreement has been submitted, it is reviewed by the Project Manager and Contract Manager
for technical clarity and accuracy. The agreement is returned to the contractor for revisions and the
review cycle continues until a final report is accepted. The Quality Assurance Manager is responsible for
developing and maintaining a program of assessment and surveillance of projects and internal processes to
ensure that they conform to the Project Specifications. Tests and Inspections are documented in reports
and corrective actions as required. The results of assessments and other surveillance activities are
compiled and presented as an element of Evomec’s yearly review process. Assessments will be planned,
scheduled, and performed in accordance with Client’s specifications.

Evomec quality system is reviewed every year, not later than the last working day of the month of

December. The review is conducted at the request of the Executive Director or Quality Assurance
Manager and involves the Quality Assurance Manager and key members of the Evomec’s Management
staff. The management review includes consideration of the effectiveness of the actions taken
subsequent to previous reviews, and an examination of corrective actions since the last review.
Amendments to the quality system to increase effectiveness are also considered. Actions required to
correct specified deficiencies are assigned to committees and management representatives. Minutes of
the meeting are taken by the Quality Assurance Manager and are retained for a minimum of 3 years.

1. Material and Labour Management

The estimated materials brought on site for the construction and installation of the various Project
components (e.g. Bored piles, steel Erections and foundations) are as detailed in the project MTO

Hazardous materials to be used during the course of construction are related to fuels, lubricants and
fluids that are required for use in construction equipment. These materials will be stored in
appropriate storage units during the construction phase of the Project by the Evomec Material Officer.
Designated storage unit areas and the type of storage units will be confirmed by the Saipem prior to
construction. Fueling of construction vehicles will take place within designated fuelling areas.

It is envisioned that 50% of the peak labour force shall be supplied through local and neighboring
communities. On-site construction activities will be limited to daylight hours, unless approved by the
County.

2. Waste Material Disposal

Waste materials brought to the site that will require removal include; consumable packaging, scraps,
fuels and other lubricants and will require reuse, recycling, and/or disposal at an appropriate
Saipem-approved off-site facility.

Sanitary waste generated during the construction phase will be collected via portable toilets and wash
stations supplied by a licensed third party who will be retained prior to the start of major construction

activities. The excavated area for the foundations and other infrastructure will consist of surface and
subsurface materials. These materials excluding excavated soil will of large dump trucks that are
capable of transporting heavy loads of excavated material. The exact type of truck and number of
truck trips required for the removal of stones and other excavated material shall be determined and
confirmed by Evomec Team prior to construction of the Project. The excavated soil removed for
installation of foundation structures shall be re-used on site as feasible. If not feasible, the soil will
be disposed of at Saipem-approved off- site facility.
Should contaminated soil be encountered during the course of excavations, the contaminated material
will be disposed of in accordance with the Waste Management Regulation.

There shall be no long-term on-site storage of waste during the construction of the Project.

11. Timing and Construction Plans

During the construction of the Project, Evomec can provide continuous updates of construction
activities to the Client to ensure staffs is aware of daily construction activities.

12. Site Communication Plans


Purpose of the Communications Plan

This plan details the planned communications efforts throughout the piling construction of the
OTUMARA Gas Processing Facility.

Goals of the Communications Plan

I. This plan intends to ensure that those within the facility have timely, accurate, and complete
information about the project through its many steps and milestones.
II. Satisfy all commitments made by Evomec to the community during the permitting phase of the
project.
III. Maintain transparency and report to Client as the project progresses.

Communication Objectives

I. Respond to 100 percent of general Client project inquiries within one business day during the 139-
days construction period.
II. Respond to 100 percent of client complaints within four hours during the 36- month construction

Non-Emergency Situations:

Evomec CLO will confirm receipt via phone/email within 24 hour period and make best efforts to
follow-up in most instances within a 5 business day period with an action plan to address or resolveany

Two way communication systems shall be maintained on site by the use of radio system and efficient

13. Emergency Procedure

When the fire alarm sounds, take the following action.

I. Evacuate immediately at the closest exit.


II. When leaving the building, close all windows and doors. This will stop the spread of fire.
III. If you encounter smoke, drop down to the floor and stay low until you reach the exit.
IV. When evacuating the office building, always use the stairs, never use the elevator, because power

Shelter in Place Procedures Sometimes "Shelter in Place" is used for emergencies for Terrorism
related incidents such as Militancy attacks and Banditry. Also, if there is an emergency within the
construction site such as a gas leak, remaining in place that is safer.

I. In case of a militancy attack or a related incident, lie low if you’re in the house boats until you
hear the all clear alarm. Stay away from the window to avoid being hit by flying bullets. .
II. Wait for the “All clear signal” from Emergency Coordinator to leave building.
III. List areas to be used for "Shelter in Place"

1) Fire Prevention Plan

Refer to Emergency Evacuation Plan.

All Evomec staff shall be inducted to Know where Fire extinguishers are located through-out the
work area.

 All staff shall be subjected toFire Extinguisher training.


 Evomec shall provide 3' allowance around all electrical equipment, fire protection devices, heating
equipment including kitchen appliances.
 44" clearances in all aisles to all exits at all times shall be maintainde.
 All staff of Evomec shall be educated to know the Areas of Refuge and purpose.
 Secondary and emergency exit routes shall be communicated to all staff.

Qualified personnel shall be provided to serve as an on-site fire watch. The sole duty of fire-watch
personnel shall be to watch for the occurrence of fire.

A fire watch shall consist of trained personnel who continuously patrol the work area. Ready
access to fire extinguishers and the ability to promptly notify project team in case of any fire
outbreak and also to implement mitigation measures.

During the patrol of the area, the fire watch officer shall not only be looking for fire, but also
making sure that the other fire protection features of the site such as egress routes and proper
lighting are available and functioning properly.

Fire watch personnel shall make continuous walking rounds through project site until work is over
daily.

Fire watch personnel shall carry a functional cellular phone while on duty, and be familiar with
the emergency number for Evomec /Saipem Operations.

Fire Watch personnel must check-in with either the manager or supervisor of the Project facility
that is performing the fire watch or the Saipem Project Supervisor, where the routine check has
been determined to be necessary by Client’s management. This check-in must occur at the
beginning and ending of each shift.
