Investigations
Hemogram, ESR, and serum investigations such as RA test, uric acid, ANA, and
HLAB-27
Nerve conduction studies and electromyogram
Plain X-rays: look for Haglund’s deformity, heel spur, osteoporosis, old fractures,
tumors, etc
USG: posterior heel pathologies
MRI: posterior heel pathologies, stress fracture, plantar fasciitis
Bone scan: stress fracture

Diagnosis 2—Plantar heel pain due to distal plantar fasciitis with gout and associ-
ated posttrauma midfoot deformity.
Management
Treatment is directed at etiological diagnosis!
The three-step protocol of heel pain management are as follows:

Rarely local steroids—Thumb rule is to inject dorsal to spur and never inject in
plantar fascia or in heel fat pad. Place the needle superior to fascia from the me-
dial side so that the solution spreads across fascial layer and avoids nerves and
fat pad. (Fig. 10.5)

Supine position with lowering of head at the end of a table will help a lot
Use of tourniquet, 2.5 magnification, and strong illumination at foot end is man-
datory
Ligation of all varicosities encountered is very important
Retraction of abductor hallucis muscle dorsally and plantarward is a must for
complete neural decompression
Use freer dissector to confirm precise decompression
Rectangular piece of medial plantar fascia is excised
Spur may not be the cause of pain, but at surgery it may be worthwhile to excise
heel spur. In the context of developing countries, most patients have a set notion
of spur being the cause of heel pain
Always release tourniquet to check for capillary feeling of decompressed nerves
and for meticulous hemostasis
Early postoperative mobilization is the key to success
In some noncompliant patients, a post-suture-removal below knee walking plas-
ter cast may help to gain confidence
Postoperative neuritic pain is expected to last up to 3 to 4 months
Surgery can be done through endoscopic method but this procedure has a steep
learning curve
