Unit Day 1

Objective: Given the physical properties of the states of matter, TSW identify physical
properties of matter correctly.

Formative Assessment: Exit slip (States of matter) will be given to the students at the end of the
lesson.

Differentiated Exit Slip for the Two Students


Science Unit Day 2:
Objective: Given the water experiment, TSW explain a question about changing states of matter
from one state to another accurately.
Formative Assessment: Provided with an exit slip at the end of the lesson, STW participate in
discussions, explorations, completion of data tables with explanations and conclusions, and
answers the questions on the exit slip.
Exit Slip - Sticky Notes: TTW write the exit slip question on the board. The questions are:
After it rains, what do you think happens to the puddles? Where does the water go?
At the end of the lesson, before students leave TTW give them sticky note exit slip to write their
answer.
Differentiated Exit Slip – Sticky Notes: TTW provide different exit slip question for students
with learning disabilities and ELL’s. The question is:
What is an example of a solid that can melt into a liquid? Draw your answer.
Science Day 3:
Objective: Given an exit slip, TSW identify the three states of matter by exposing liquids to a
heat source to change them to gases and by exposing them to frozen temperatures to change them
to solid correctly by recalling from a science experiment.
Formative Assessment: Exit slip will be given to the students.
