MEMORANDUM OF AGREEMENT

KNOW ALL MEN AND BY THESE PRESENTS:

This agreement made and entered into this _____ day of __________________________, 2018
between:
CARITAS MANILA, INC., represented by its Executive Director, REV. FR. ANTON C.T.
PASCUAL with office address at 2002 Jesus St., Pandacan, 1011 Manila, hereinafter referred to
as the FIRST PARTY;

In Partnership With

Diocesan Shrine and Parish of Our Lady of the Holy Rosary represented by its Parish Priest,
REV. FR. VICENTICO C. FLORES, JR. with office address at Rizal St., Barangay Real,
Cardona, Rizal and Asian Social Institute, represented by its Vice President, Mr. Dennis Y.
Batoy, with office address at 1518 Leon Guinto Sr. Malate Manila, hereinafter referred to as the
SECOND PARTY;

Whereas the FIRST PARTY is the social development arm of the Archdiocese of Manila, its
programs and services are directed towards the total human development of marginalized people
and the promotion of social justice in the light of the Catholic Social Teaching.

Whereas the SECOND PARTY are the Diocesan Shrine and Parish of Our Lady of the Holy
Rosary under the Diocese of Antipolo and the Social Development Department of Asian Social
Institute therewith.

Now therefore, for and in consideration of the foregoing premises, all parties hereto hereby agree
to the following:

I. DUTIES AND OBLIGATIONS

A. FIRST PARTY
A1. Shall approve recommended applicants of the Second Party according to set
criteria and policy.
A2. Shall provide technical assistance to the Second Party as capacity building
in implementing the Youth Servant Leadership and Education Program.
A3. Shall provide integrated youth development program towards servant
leadership to qualified youth-partners of the Second Party.
A4.1. Shall provide financial assistance in the form of monthly allowance (up to
maximum of Php. 1, 500.00 monthly for 10 months only) and shall be
deposited under the account of the YSLs BPI account or Implementing
Caritas Partner (apply to ICPs in Suffragan Dioceses with no counterpart
amounting to 5000 as per required).
A4.2. Shall provide financial assistance in the form of monthly allowance (up to
maximum Php. 2, 000.00 monthly for 10 months only) and shall be
deposited
under the account of the YSLs BPI account or thru the ICPs official bank
 account.
A5. Shall forfeit the monthly allowance in case of failure to submit all required
documents, monthly and renewal reports on time and not following the
standard as required by the program.
A6. Shall facilitate and conduct an Audit of the program anytime which include
financial and operations audit.

B. SECOND PARTY

B1. Shall screen and recommend deserving beneficiaries of Youth Servant

Leadership and Education Program based on set criteria and guidelines of the
First Party:

 Applicant must be an active servant volunteer/leader of the

church/community.
 Average grade must be at least 85%.
 Applicant must come from poor or indigent family.
 The family of the applicant is supportive of the education of their child.
 The family is willing to undergo integrated development program i.e.
Formation, Health and Nutrition Seminars, Sacramental Appreciation
and Livelihood Training Program.
B2. Shall provide fund or budget for the operating cost while implementing
YSLEP
in the respective area or diocese, which include transportation and
accommodation of YSLEP Team (Staff/Volunteers), communication,
reportorial requirements, monitoring and evaluation meetings, conduct of
trainings and formation of YSLEP Team (Staff/Volunteers) and Youth
Servant Leaders, and organizing Annual KAIROS and other activities of
YSLEP under its regular cycle.
B3. Shall have an institution’s bank account for fund transfer transactions
(Optional
MM).
B4. Shall facilitate Church involvement of YSLs from orientation about the
Church
and Ministry up to proper deployment to ministries and organizations
pertaining to youth development as stated in the YSLEP policy.
B5. Shall facilitate monthly YSLEP formation and meeting of partner Youth
Servant Leaders.
B6. Shall be responsible for regular monitoring and evaluation of partner
Youth Servant Leaders and provides recommendation to Caritas for action.
B7. Shall provide the First Party of monthly and semestral report on the status of
Youth Servant Leaders in terms of Formation attendance, academic
performance, Church involvement and social apostolate.
B8. Shall provide opportunity to the youth and their families with developmental
programs and services i.e. Livelihood, values formation and sacramental
appreciation.
B9. In case of involuntary withdrawal of the YSL from the program without due
cause and termination due to misconduct of the YSL, the ICP must return the
total amount of financial assistance given by Caritas Manila to the YSL from
the start of the program up to the last financial transaction covered by the
program.
 B10. Shall be required to submit an annual plan and schedule of social apostolate,
church involvement activities and YSLEP formation beginning of each year
upon approval and renewal of partnership.
B11. Shall be required to submit liquidation of every released allowance together
with the monthly monitoring report on the status of YSLs on the major
components of YSLEP with the attached monthly attendance sheet and
minutes of the meeting before the release of the following monthly
allowance.

II. EFFECTIVITY

This Memorandum of Agreement shall be deemed effective this school year 2018-
2019 and upon signing of the authorized representatives of the contracting parties of
the MOA. Any violation of this contract shall be enough cause of its suspension,
revocation or cancellation. There shall be a thirty-day (30) notice before the
effectivity of any termination.
IN WITNESS WHEREOF, the parties have hereunto affixed their signature this _____ day
