1. Original Report Card (Form 138) from Senior High School
2. Original Birth Certificate from Philippine Statistics Authority (PSA)
3. Two (2) copies of 2 x 2 ID photographs (colored with white background)
4. Ballpen and pencil
5. College Admission Examination Fee Php500.00

February 23, 2019: Fr. Saturnino Urios University, Butuan City, Agusan del Norte
March 2, 2019: St. Paul School, Ormoc City, Leyte
March 2, 2019: Holy Spirit School, Tagbilaran City, Bohol
March 16, 2019: Liceo del Verbo Divino (LVD), Tacloban City, Leyte
To all USC tertiary freshmen applicants who will be taking the USC College Admission
Examination (CAE) in the above Testing Centers, please register online at
ismis.usc.edu.ph/apply. Pay the CAE fee through the following payment channels:
1. UnionBank
2. Cebuana Lhuillier
3. Prince Retail
4. Bank of the Philippine Islands
Instruction for payment:
1. Pay the required amount via bills payment/payment facility
2. Merchant name: UNIVERSITY OF SAN CARLOS / USCARL
3. Reference number: 6900 + Name of examinee

Submit VALIDATED PAYMENT SLIP with the requirements one day prior to the testing
schedule at the testing centers enumerated above.

Cut-off dates for Admission and CAE

May 25, 2019: For non-STEM applicants who plan to enroll in Engineering programs
May 25, 2019: For non-ABM applicants who plan to enroll in Business and Economics programs
July 27, 2019: For other applicants not specified above
