AFFIDAVIT OF DISCREPANCY

I, BRIAN DENN AGUILAR YUTUC, of legal age, Filipino citizen, married and with
address at Barangay Abogado, Paniqui, Tarlac, after having been duly sworn to according to law
hereby depose and state:

1. That I was born on September 13, 1981 in San Anotnio, Zambales, to Spouses
Redentor B. Yutuc and Anilene P. Aguilar;

2. That my birth was registered at the Office of the Local Civil Registrar of San
Anotnio, Zambales;

3. That as per DECISION of my adoption paper with SP. PROC. NO. 4210, issued by
the Regional Trial Court, Third Judicial Region Branch 48, San Fernando,
Pampanga, the entry with respect to my first name was erroneously entered as
BRIAN DEAN, instead of BRIAN DENN, which is correct, as appearing in my
Certificate of Live Birth and all other pertinent papers and documents;

4. That the error as regards to my first name in said adoption paper was due to
inadvertence or typographical error in typing the said entry, without any malicious
intent or purpose and was not done in bad faith;

5. That the names BRIAN DEAN YUTUC, as appearing in the DECISION of my

adoption paper and BRIAN DENN AGUILAR YUTUC, as appearing in my
Certificate of Live Birth and other pertinent documents, refers to one and the same
person;

6. That I am executing this affidavit in order to attest to the truth of the foregoing
circumstances, and for all legal intents and purposes this may serve.
