Separate the COA for the Following:

• Civil Works (Per area and/or scope)

> Pavement
> Curbs & Gutter
> Side Walks
• MEPFS System
> Waterline System
> Fire Protection Line
> Sewer Line
> Drainage System

Attachment of the aforementioned per COA should have the following:

• Accomplished T&C results :

> Pre-functional Checklist
> Pre-functional Test
> Final Performance test & Commissioning
• Punchlist Close out forms
• A4 Size as-built plan with highlight of the area proposing to be accepted
• A3 Size complete set as-built plans
• Calibration Certificate (Water meter's and etc)
• Contractor's warranty to MDC
• Warranty certificate of MDC to SBU
• Operation Instruction & Maintenance (OIM)
• Contact information of Supplier
• Other documents:
> Listings and Certificate of installed underground valves (if applicable)
> Listings and Certificate of installed Fire hydrants (if applicable)
> Compact certificate

(2) COA front page (to be signed by)

• Replace the "Approved by" to "Noted by" on APMC Signatories

• Follow the below format and position of the following APMC signatories
