AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, ESTER R. VELEÑA and ALFREDA P. LARGO, both of legal ages,
Filipinos, and residents of Barangay Caayunan, Basud, Camarines Norte,
respectively, after having been duly sworn to in accordance with law, hereby
depose and say:

That we personally know the deceased MARIA C. NISORTADO and her

family, having been friends/neighbors with them for a very long time;

That Maria died on February 23, 2020 at Basud, Camarines Norte;

That we know for a fact that her correct surname is NISORTADO;

That during her lifetime she has been using the said surname in all of
her personal dealings and transactions;

That, however, in the records of Office of Senior Citizen Affairs (OSCA),

Basud, Camarines Norte, her surname is erroneously appears as Español;

That the erroneous entry of her surname in records of the Office of

Senior Citizen Affairs (OSCA), Basud, Camarines Norte is merely a
typographical or clerical error;

That the person appearing in records of Office of Senior Citizen Affairs

(OSCA), Basud, Camarines Norte, as having the erroneous surname of Español
and the MARIA C. NISORTADO that we person nally know refers to one and
the same person;

That we are not in any way related to the deceased MARIA C.

NISORTADO;

That we are executing this joint affidavit to attest to the truth of the
foregoing statements and for whatever legal purpose this instrument may
serve.
