Pipot : “Good morning... Welcome to Notokusumo Clinic. I am nurse Fitriyah. Can I Help
you?”
Aulia : “Good morning... I want to check-up.”
Pipot : “What’s your name, Mrs?
Auli : “My name is Mrs. Aulia.”
Pipot : “Please sit down, Mrs. Aulia. What’s your problem?”
Aulia : “I have no serious problem, but I just want to check it out.”
Pipot : “Ok Mrs. Have you come here before?”
Aulia : “It’s the first time I come to this clinic.”
Pipot : “OK. We always provide the best service to our patient. What Kind of medical
check-up do you want?”
Aulia : “I want to check it in it’s entirety, from the basic to the most complete.”
Pipot : “Alrigh Mrs. Aulia we will prepare it. Is there anything you want to ask?
Aulia : “How about the fee?”
Pipot : “For the cost you have to pay IDR 800.000,- .”
Aulia : “Ok, Nurse.”
Pipot : “Before it, you should fill the registration from, Mrs.”
Aulia : “Alright.”

A few minutes later

Pipot : “Have you done Mrs?”

Aulia : “I have done, Nurse.”
Pipot : “Ok, I will check it first. So, your name is Mrs.Aulia Nur Darmawanti, your
date of birth is 5th of August 1998, you want to check up, is that right?”
Aulia : “Yes, right.”
Pipot : “I’ll input your data to the computer. You can wait in the waiting room, until
your name called by nurse again. Is there anything I can help again?”
Aulia : “What should I do after this?"
Pipot : “ There is no, you can come back here tomorrow. Relax for the examination. I
hope you always healthy, Mrs.”
Aulia : “Thank you, Nurse.”
Pipot : “Your welcome.”

Nurse   :  Good morning.

Patient :  Good morning nurse. I want to check up.
Nurse   :  Yes, come in please. I'm sorry, how can I call you?
Patient :  You can call me Mrs. Aulia
Nurse   :  My name's Fitriyah, I'm registered nurse, and I'll be looking after you today. What are
you feeling now?
Patient :  I’m well nurse, I just want to check-up.
Nurse   : We will examine you. Before it, now I've got some questions I need to ask you, and
some admission card. It's a personal data for your admission. Are you ready?
Patient :  Yes, of course.
Nurse   :  Could you tell me your complete name?
Patient :  My name is Aulia Nur.
Nurse   :  Could you tell me your nickname?
Patient :  My nickname is Aulia
Nurse   :  How old are you?
Patient :  I'm 20 years old.
Nurse   :  Where do you live?
Patient :  I live in Yogyakarta
Nurse   : And then what's your religion?
Patient :  I'm a Moslem nurse
Nurse   :  When were you born?
Patient :  I was born in Yogyakarta on 5th August 1998
Nurse   :  Okay Mrs. Aulia. Please have a seat while waiting until we call your name.

Nurse   :  Mrs. Aulia!

Patient :  Yes nurse, where should I go?
Nurse   :  You can go ahead and than take the second hall on the right and you will see the room
to check your health.
Patient :  Thank you.
Nurse : your welcome.
