Dear Mr. Bragat;

This refers to the quarterly submission of Self-Monitoring Reports (SMR) pursuant to

DENR Administrative Order No. 27 series of 2003 wherein firms with EMB permits shall
submit fifteen (15) days from the end of each quarter a notarized SMR covering all activities
in the quarter.

Please be informed that due to the advent of the Corona Virus pandemic we now facing,
a memorandum dated 01 April 2020 was issued by the OIC-EMB Director instructing all
regulated industrial/commercial establishments, including hospitals and healthcare facilities to
install adequate disinfection at the last stage of the wastewater treatment facility to eliminate
or remove the COVID-19 virus within your establishment and immediate environment.
Moreover, a memorandum was issued by the OIC-EMB Director on April 14, 2020 instructing
all EMB Regional Offices to gather data on the alternative modes of treatment and disposal of
pathological and infectious healthcare wastes (M501) being practiced by your facility within
this period.

In line with these, please submit your duly signed 1 st Quarter Self-Monitoring Report
on or before 20 April 2020 without the need for notarization provided however, that the
notarized page will be submitted after the Enhanced Community Quarantine is lifted.
Information regarding the amount of generated wastewater and infectious wastes, disinfection
and mitigation processes, and other waste management practices must be included in the report.
