I, ROENTGEN RUEL R. RABE, of legal age, Filipino, married to Aimee Rachel D. Rabe,
and presently residing at E. Jacinto St. Caballareza Compound, Brgy. 8, Lucban, Quezon,
after being duly sworn to in accordance with law do hereby depose and say:
1) That I am the father of minor child BIANCA YSABEL D. RABE, born on April
16, 2002;
2) That I am giving my consent for my child to travel abroad to be accompanied by
her teachers and professors, Dr. Victor Emmanuel Carmelo D. Nadera Jr., Christian
Tablazon, and Jon Darren Rivera;
3) That I guarantee that they will not violate any existing laws of their country of
destination (SINGAPORE) and
4) That I am executing this affidavit in connection with my child’s application for a
passport at the Department of Foreign Affairs and/or travel clearance from the Department of
Social Welfare and Development for their travel abroad.
