Addon Credit Card Application – Important Points

 Add-on card can only be issued for any of the following relations: Parent / Spouse / Child /
Brother / Sister / Father-in-law / Mother-in-law / Daughter-in-law / Son-in-law / Brother-in-law /
Sister-in-law.
 Issuance of Credit Card is at the sole discretion of the bank and is subject to bank's internal
approval criteria.
 The additional card applicant should reside at the same address as the primary card holder.
 Upto 5 additional cards can be availed at no additional cost*
 Addon card will have common credit limit along with primary card and one consolidated
statement will be available for all purchases on primary and addon card
 Following documents will be required for the addon application:
o Duly filled application form. Please download the addon application form here.
o Copy of self-attested ID proof (Valid Passport / Driver's License/Voter’s ID Card /Aadhaar
Card)
o Passport size photograph
