1. You must be a Singaporean Citizen or Singapore Permanent Resident
2. Your monthly income has been reduced by 25% or more after 1 February 2020 due to the Covid-19 outbreak
3. Your existing Maybank Credit Card and/or CreditAble account(s) are between 30 and 90 days past due as at
application date
4. You must not be on any existing debt repayment or restructuring program with Maybank (including Debt Repayment
Plan, Debt Management Program, Debt Consolidation Plan and Repayment Assistance Scheme)

4 DECLARATION & AUTHORISATION
By signing this SFRP (Unsecured) Application form:

1. I agree that: -
(a) The entire outstanding balances on my Maybank Card(s) and CreditAble, including remaining interest-free
instalment payment plans, instalment loans, fees & charges determined by the bank at the point of application
will be converted to a term loan.
(b) My credit card(s) accounts will be closed upon approval of the SFRP (Unsecured) and all TREATS reward points
will be forfeited.
(c) Any recurring arrangements on my credit card(s) will be blocked and alternative payment arrangements will
be made.
(d) All outstanding balances will be converted to a term loan under a line of credit facility, CreditAble. A new
CreditAble account will be open if I do not have any existing line of credit facility.
(e) I will not be able to draw down on any unutilised limit on my CreditAble account.
(f) The monthly minimum payment due may exceed 3% of the total loan amount
(g) Interest and late charges as determined by the bank will apply if full monthly instalment amount is not
received.
(h) All other CreditAble Terms & Conditions apply.

2. I represent and warrant that: -

(a) All information and documents provided above or otherwise in connection with this application are true and
complete in all respect and that I have not withheld any information and/or documents which may be material
in the context of this application.
(b) If there is a change in the information provided and/or representations given or the information and/or
representations become inaccurate in any way, I shall promptly notify you of the change or inaccuracy.

3. I consent and authorise: -

(a) you to collect, use, process and/or disclose my Personal Data to Maybank’s authorised service providers and
relevant third parties for purposes reasonably required by Maybank to process my/our application(s), provide
