Respected Sir

Please accept the formal notice of my resignation from the position of Senior Embryologist with effect
from 01-09-2015. As I know my responsibility for our very busy centre I am happy to continue to work
until 31-10-2015.

I have been working for the Isha Hospital from 15/07/2013. While I believe that I am moving for
personal reason. I am sorry to leave one of the best Fertility Centre and I thank you for your support
during my tenure which I found enjoyable and fulfilling. I am also thankful for the hospital management
for providing me such good work atmosphere allowing me to grow professionally. I sincerely regret to
leave such good place.
