    FDA expects applicants to follow the provided requirements when applying for

     Application form

     Certificate of accreditation and registration
     List of products
     A sample of the product for the presentation
     Labels and labeling materials used
     Certificate of Analysis
     Flow method of manufacturing
     Registration fee
