AFFIDAVIT

I, ANNA MICHAELA URQUIOLA FRANCO of legal age, single, Filipino

citizen, married, with residence address at No. 30 Sandiko St., BF Homes,
Barangay Holy Spirit, Quezon City, after having been duly sworn to in
accordance with law, hereby depose and state that:

1. At the age of 20 I gave birth to my son, Michael Gabriel Franco on

April 3, 2014 at Pasig City, Philippines;

2. I and the biological father of my son John Robert Maquirang Palomo

never got married;

3. In fact since we have separated ways in December of 2014, On April

3, 2015 he went to our house he just dropped his gift to our son and
our house helper received the said gift because we were not in our
house, as we were celebrating Michael’s birthday, after that we
have no communication as such I have also never received any
financial support from him;

4. Being the biological mother of my son I have full custody of him in

pursuance to the Family Code of the Philippines;

5. Due to this circumstance I solely raised our child and fully support
and provide for all of his needs without the help of his father;

6. At present I am employed as an OFW in Hong Kong and with postal

address at Flat 806 Block 5B Century Link Tung Chung Hong Kong;
and

7. That I execute this affidavit to attest to the truth and veracity of the
above foregoing facts and for all other legal purposes it may serve;

IN WITNESS WHEREOF, I have hereunto set my hand this _____ day of
