About Disability
The disability board of GMCH, Sector 32, Chandigarh has the following constituted members.

» Dr.Arvind Malhotra: Assistant Professor, Deptt. of Neurology : Member

» Dr.Nitin Gupta : Assistant Profesoor, Deptt. of ENT : Member

The persons with disability from U.T. Chandigarh can apply for disability certificate by either having an
application form from the office of department of Ophthalmology, Room No.318, Block-D, Level III on any working
day or else it can be downloaded from the website of GMCH, www.gmch.gov.in under the Hospital Services. The duly
filled application form may be submitted in the department of Ophthalmology on any working day along with the OPD
card.

The persons with ENT disability should get themselves examined by Dr. Nitin Gupta, Assistant Professor on
Wednesday, Block B, Level III, ENT OPD after 10.30 am to 1.00 pm.

The persons with eye disability should get themselves examined by Dr. R.K. Bansal on any Tuesday, Block B,
Level IV, EYE OPD after 10.30 am to 1.00 pm.

The persons with neuro disability should get themselves examined by Dr. Arvind Malhotra on Tuesday, Block B,
Level III, NEURO OPD after 10.30 am to 1.00 pm.

The persons with ortho disability should get themselves examined by Dr. Ravi Kumar Preenja on Wednesday,
Block B, Level III, ORTHO OPD after 10.30 am to 1.00 pm.

The examination report on the outdoor ticket, application form and patient’s four passport size photographs are to
be filed with the office of the department of Ophthalmology, ‘D’ Block, Level III. Thereafter within 15 days, the
disability certificate will be ready and the candidate can receive it from the EYE office after depositing Rs.30/- for
general patients and Rs.690/- for MACT cases.

NOTE: The residence proof and identification proof should be brought at the time of submitting the application form.
