Are their steps to get to your floor? I live on the 4th floor, there is an elevator

Do you use any equipment to help you walk? I used a rolling walker both in and out of the
home.

Assistance shopping? Everything is done for me at the Atria

Getting to Dr. Appts? They take me in an ambulette to my dr. appts.
Making meals I went to the dining room for meals

Do you have a bathtub or shower? Shower

Do you have a seat/bench in the shower? Yes
Does anyone help you shower? Someone helped me wash my lower body, i
did the rest myself
Are there grab bars? Yes, by the shower and by the toilet

Do you have a raised toilet seat? Yes

 Do you have goals for therapy? I want to get back to how i was before the
stroke

Can you lift your arms up for me? *Lift up right arm fully, do not move the left

● Demonstrate eating with a fork- only eat food on the right side of the plate
● Students will instruct you how to put on a shirt- only use right arm the whole time.
