1. INCIDENTS:

2. UNSAFE ACTS:
1. During Confined Space entry in 130-V1, worker not wearing chin
strap with helmet in A-200.
2. Hot job of cutting, welding and grinding job on spool piece near
300-H1 was in progress while new Hot job work permit was not
issued.
3. Lifting of spool piece through crane was in progress. While two
workers were performing Scaffolding job on 200-E3 which was
under the boom of the crane. Area was also not barricaded.
4. Cutting welding and grinding job was in progress on process line
of 200-V2, in A-400. Near by drains were not covered properly.
5. Compressor was being used for supplying air for ventilation at
west side of A-500 near 802-V53 while near by drains were not
covered.
6. Drums were overfilled in area also waste segregation not
properly done hazardous waste present in green drum
7. Road was closed without road closure permit. DEL supervisor
Hammad having road closure permit without authorized by HSE
and without giving intimation to fire department (HSE).
8. During welding of stack spool, heavy spatters were coming out.
Fire blankets were not provided properly to cover the area near
110-H1.
9. De-bolting of Gate valves was being carried out. Helper Mohsin
was found in front of hammer near 100-P6B.
10.Two workers found on working on uncertified scaffold in A-300
(recycle scrubber knockout drum).
11.Rigger Akmal found on height without hooked up with harness
near 200-PV151.

3. UNSAFE CONDITIONS:
1. In A-400, During Bundle Pulling of 300-E2A, area below lifted
bundle extractor not barricaded and movement of worker
observed while showering of water in progress.
2. Scaffolding issue observed near 200-V1, platform planks not
properly tied and cross bracing missing while green tag at-
tached.(No Inspection date and inspector name mentioned on
the tag.
3. During material Shifting by crane, outriggers not fully extend-
ed, guiding rope not tied to spool piece near 100-V1.
4. Scaffolding at east side of cooling tower was in progress, dam-
aged/ corroded planks and tubes were being used
5. Excavation 6ft deep at east side of cooling tower was in pro-
gress, edge was of excavation was not protected
6. Poor housekeeping conditions were observed on top platform
of CDU:
7. Glass wool insulation was present. Loose scaffolding pipes and
couplers were present.
8. Air Hose pipes present in front of top man-way of 200-V03
causing access block and tripping hazard in A-400.
9. Confined space entry in Stripper Column was stopped while
during no entry in confined no sign post or barrication tape
placed at manhole.
10. Welding job was in progress on naphtha line. Canopy was not
properly arranged due to which heavy spatters were coming
out. No proper excess was arrange to approach 10 ft. height.
Welder Altaf was not safety oriented near 100-E1B.
4. MAJOR CONCERNS:
1. Oxy-acetylene cutting job was being performed while OSHA
recommended protection of eyes" Filter Lenses for Protection
Against Radiant Energy" not carried out by performer ( 29
CFR 1910.133(a)(5))
2. No fire watcher was deputed for welding and grinding job.
Also workers were using iron hammer in Main corridors of A-
500.
3. Canopies were not proper at different locations ( 110-H1,
411-E12, DHDS main corridor TP-12, 100-E1B).
