This letter intends to refer Mr. Simon into your care, who has a diagnosis of past progressing
early dementia. He will be discharged today and he will require continuous care.

With regard to social history, Mr. Simon is living with his family at 201/275 Alfred St, North
Sydney, NSW. In addition to this, despite the fact that he only speaks French, her daughter
can be his interpreter. Also, he reported that he is more active during the night and sleeps at
daytime. Good!

Looking at the patient’s medical status, Mr. Simon was admitted last 22nd of January with
several complaints such as headaches, high fever, body pain, discomfort, and poor
appetite. Upon history taking, it was noted that he has high blood pressure, depression as
well as degenerative joint disease. Good!

It would be greatly appreciated if you could accept our patient, Mr. Daniel for ongoing
management. Currently, he is taking paracetamol, 500mg thrice daily and advised to take
aspirin if there is more pain. Additionally, he is ambulatory with the utilization of assistive
device. Importantly, it was recorded that he is allergic to peanuts. Good!
