AFFIDAVIT OF DENIAL

I, BRITNEY SPEAKS, of legal age, married, Filipino, and a resident

of Barangay 130, Cagayan de Oro City, after having been duly sworn to an
oath in accordance with law, hereby depose and say:

1. That I am an applicant for a National Bureau of Investigation

(NBI) Clearance;

2. That according to the files of their Office, a person named

Britney Speaks has a case of Violation of BP 22 with
Case No. M11-0-356 to M11-02-360 filed at the Branch
004 MTC Misamis Oriental;

3. That I am not the same person charged and is currently

‘CHARGED’ in the aforementioned case;

4. That I hereby deny that I am the person named in their

record;

5. That I am executing this Affidavit to attest to the truth of the

foregoing and for whatever purposes this may serve.
