JUDICIAL AFFIDAVIT
OF ILUMINADO VICENTE

I, Iluminado Vicente, Filipino, married, of legal age, with


address at Diliman, Quezon City, state under oath as follows:

PRELIMINARY STATEMENT

The person examining me is Atty. Lyceum with address at the


180 Leviste Street, Makati City, where the examination was held. The
questions were propounded in English, a language that I fully
understand and the answers were likewise given by me in English. I
am answering his questions fully conscious that I do so under oath
and may face criminal liability for false testimony and perjury.

PURPOSE: This affidavit/testimony of Iluminado Vicente is


being offered to prove that he acted responsibly to comply with his
duties and responsibilities as sanitary inspector of Defendant
Metropolitan Water District, and that all the allegations against
Defendant and its employees, in the Complaint are neither true nor
correct.

Q1: Please state your name, address and other personal


circumstances.
A1: I am Iluminado Vicente, Filipino, married, of legal age,
with address at Diliman, Quezon City.

Q2: What is your relation to this case?
A2: I am the sanitary inspector who was called by the
Defendant Metropolitan Water District’s clinic to render assistance.

Q3: What were you able to do for the victim?


A3: Upon arriving, I injected the boy with camphorated oil,
then left on a jeep to fetch Dr. Ayuyao from the University of the
Philippines for help.

Q4: What happened after you and Dr. Ayuyao arrived?


A4: We arrived with another resuscitator, but the same
became of no use because the boy already died.
