5. Professional driver’s license

7. Proof of health examination of drivers
8. List of specific vehicles that will be used
12 Certification of the Activity Head that Parents/ Guardians’
Consent Form are secured and copies of which are filed in the
Office of the principal and given to the concerned teachers
