I understand and agree that my personal data, including sensitive personal data, may be used
by Sribhagawan Education Group Sdn. Bhd. to monitor recruitment initiatives and related
policies.

I understand and agree that my personal data, including sensitive personal data, may be
disclosed to third parties to verify or obtain additional information including education
institutions, current/previous employers and credit reference agencies. I understand that even if
my application is unsuccessful, my personal data, including sensitive personal data, may be
retained for consideration for future job opportunities in the Company and/or any of its
subsidiaries.

I voluntarily give Sribhagawan Education Group Sdn. Bhd. the right to make a thorough
investigation of my past employment and activities, agree to co-operate in such investigation,
and release from all liability or responsibility all persons, companies or corporations supplying
such information. I hereby consent to all persons, companies or corporations sharing any
information obtained about me in order for Sribhagawan Education Group Sdn. Bhd. to consider
making an offer of employment to me. Specifically, I hereby give my explicit consent to all
persons, companies or corporations to share my sensitive personal data for the same purpose.

I confirm that I have duly obtained consent of my current and previous employers, my spouse,
my children, my parents, my referees, and/or any other third parties contained herein for the use
and disclosure of their personal data, including obtaining explicit consent on their sensitive
personal data to be processed by the Company in accordance with this application.

By executing this form, I agree that the entire contents of the Employment Application Form, as
well as the report of any such examination, may be used by the Company for purposes of
considering my job application. Specifically, further to the Personal Data Protection Act 2010, I
hereby consent to the processing of my personal data, including giving my explicit consent for
the processing of my sensitive personal data including financial and legal status if require by
Sribhagawan Education Group Sdn. Bhd. and/or any of its subsidiaries, business partners,
associate companies and/or third parties for purposes of considering my job application.

Pursuant to the Credit Reporting Agencies (CRA) Act 2010 and Central Bank of Malaysia Act
2009, I the undersigned do hereby give my consent to you and CTOS Data Systems Sdn Bhd
(CTOS) , a registered credit reporting agency under the CRA Act to process my personal data.

By this consent, I understand and agree that :

i) You may conduct credit/trade check, CCRIS and DCHEQS checks on me and where
applicable with CTOS at any time for as long as I have a trade relationship with you or
where any dues remain unpaid and outstanding with you, for any one or more of the
following purposes :

√ Opening of account
√ Debt recovery
√ Credit/Account review
√ Legal documentation consequent to a contract or facility granted by you.
√ Credit/Account monitoring
√ Credit/Account evaluation

ii) You may disclose any information on my conduct of my account(s) with you, to any
business entity/ies for bona fide trade checking at any time. I am also aware and
understand that such information will be provided to CTOS, who may in turn share such
information to subscribers of their service.

iii) Where you require any processing of my application to be processed by any processing
centre located outside Malaysia (including your Head Office), I hereby give consent to
CTOS to disclose my credit, CCRIS & DCHEQS reports to such locations outside
Malaysia.

iv) Apart from the above, I the undersigned do give my consent to you and the CTOS, to
process my personal data as per the PDPA Act.
