Neurologic Exam:

CEREBRAL FUNCTION
Patient is stuporous, aphasic, speech is slurred but understandable. No data gathered regarding orientation
to time, place and person, calculation of simple mathematical problems, recollection of remote, recent and
immediate memories, general information and judgment due to uncooperative patient.

CRANIAL NERVES
I- NA
II- Not performed
III- Direct & consensual pupillary light reflexes were elicited with slightly sluggish response with the
right eye
IV, V, VI- able to follow the 6 cardinal movements of EOM
V-able to identify dull & sharp sensation, not able to clench & open jaw
VII- Facial asymmetry noted on the right side
VIII-able to hear whispered words, localize the sound symmetrically
IX&X- Not performed
IX- Hoarseness of the voice noted
XI- NA
XII-NA

MOTOR TESTING
No atrophy/hypertrophy, involuntary movements, or fasciculation noted. 
Normotonic muscle. No spasticity or rigidity noted on the upper & lower extremities.
Muscle strength on the right arm is 2/5, 2/5 on the right lower extremity, and 3/5 on the left arm and left
lower extremity.

CEREBELLAR FUNCTION
Unable to perform

REFLEXES
+1= left upper (biceps&triceps) & lower (knee&ankle)
0 = right lower(knee and ankle)
0 = right upper(biceps&triceps)
(-) Babinski

SENSORY TESTING
Able to identify sharp and dull stimulus symmetrically on the upper and lower extemities but needed
repetitive stimulus on the right side of the face before able to finally identify it. 
Withdraws to pain. Stereognosis test not performed. Rhomberg's test not performed.

MENINGEAL SIGNS
Neck is supple. (-) Kernig's & Brudzinski test
