Course Description : This course deals with concepts, principles, theories and techniques of history taking, head to toe physical examination and psychosocial
assessment using various tools and interpretation of laboratory findings to arrive at a nursing diagnosis. The learners are expected to perform holistic nursing assessment of an individual adult client.

IX. COURSE POLICIES AND STANDARDS
1. A student who missed a quiz, pretest/posttest, and/or long quiz will be allowed to take the quiz if he/she has valid reason for being absent.

2. If a student failed to take major exams on scheduled time, he/she has to submit a letter explaining the reason for not taking it signed by parent/guardian and must pay in cashier for special examination fee. The
student should bring the Official Receipt (OR) and present it to the teacher in charge before taking the examination.

3. All requirements should be passed on or before the set deadline; failure to do so means a grade of 50% for the said requirement.

4. Students should not represent the work of others as their own. Respect the intellectual property rights of others. Plagiarism is a major offense.
5. Absences for which medical or court excuse is provided will be recorded but not figured in the attendance grade. Likewise, an absence for which advance notice is given by phone or in person will not be figured
in attendance grade if he presents a valid excuse letter upon coming back to class. Tardiness or early departure from class will be figured as a half absence. A student who has ten (10) hours successive
unexcused absences will be dropped from the course. Behavior is graded to the observation of the instructor.

For RLE Skills Laboratory

6. Tardiness in Clinical Area/RLE Skills Laboratory means absent after 30 minutes arrival from the scheduled class

7. 3 lates is equivalent to 1 day unexcused absent

8. Students are only allowed 2 unexcused absences. If a student had an absences beyond 3 unexcused days, he/she will be on probation for dismissal from the program.

9. Students are only allowed 9 make up duties per semester.

10. If a student failed to conduct a return demo on scheduled time, he/she has to submit a letter explaining the reason for not perform it signed by parent/guardian. However, if there is no valid reason for being
absent, he/she forfeited his/her chance to perform and will be given a grade of 50%.

X. CLASSROOM POLICIES
1. Students has to wear complete school uniform in attending major and professional subject (with Student ID and Nameplate)

2. Students should wear proper uniform.

3. No cellular phones or music players are allowed for use in the classroom/Clinical Area unless it is required.

4. The use of a head set during class is prohibited.

5. Eating during classes/Duty hours is prohibited not unless allowed by the Classroom/Clinical Instructors.

NOTE: Non-compliance to classroom policies will be given an Incidental Report (IR). One incident is equal to one (1) IR. Non submission of Filled out IR Form to assigned classroom and/or Clinical Instructor
will require to submit two (2) filled out IR forms.

XI. EMERGENCY PROCEDURES


1. Evacuation procedures – instructions are posted in some areas that can be seen right away. Inform the instructor right away if accidents happen in the course of the activity.

XII. SUGGESTIONS FOR SUCCESS

For a nursing student to be a better nurse someday, she must have a strong stanchion of the basics first. Thus, a dedicated review of this course will allow you to be committed to nursing and will help you practice
theory-based interventions. Furthermore, it is encouraged to participate in every endeavor inside and outside the classroom to develop self-awareness and enhance oneself.
