AFFIDAVIT OF NON-REVOCABLE RETIREMENT

I, PO2 PROCESO FERNANDEZ MARCOS , with Badge No. 109270, presently

assigned at Gattaran Police Station, Gattaran Cagayan, of legal age, Filipino, and a
resident of Centro Norte Sto. Niño, Cagayan, after having been duly sworn to in
accordance with law do hereby depose and state:

1. That I will be filing my application for Optional Retirement with the PNP
service pursuant to Section 40 of RA 6975;

2. That the effective date of my Optional Retirement is on 31 March 2018;

3. That once the order will be issued in connection with my Optional

Retirement, I will not request nor ask for the revocation of the same;

4. That I am executing this document freely and voluntarily, and to support

the issuance of my Retirement Order.
