From the statement of facts raised by the chief engineer on board, the following is the summary of
events:
Location : Campos Basin Static Towing
Today May 5 on 18.05 during on static operation with FPSO I was informed by 3E about high level
oil in the crankcase of Stbd ME as the result was overflow. 18.20 I’m informed master and
requested to stop engine to pump out oil to rich normal levels. 18.30 ME has been stopped.
Before stopped ME running OK and no any alarms. 18.55 was open the crankcase door for
pumping oil and crankcase inspection. During inspection were found some damages and metal
parts inside crankcase:
1. the gear wheel is clamped to the crankshaft by the conical collar flange with the
screws. One screw was lost due to unscrew and found inside;
3. the oil pipe line for the lubrication broken and cut, found inside.

At the time of the breakdown the vessel was engaged in operations with tankers and there was no
possibility to disengage.
The engine was stopped by chief engineer did not stopped by emergency shutdown.
Due the nature of the damages the vessel was instructed to head for Rio De Janeiro on completion
of the task at hand to receive shore assistance from Rolls Royce personal and perform the required
repairs including the renewal of the damaged gears
Vessel arrived at anchorage on 13th May 2014 @07300 hrs and proceeds with repairs

1. Request and obtain assembly procedures from manufacturer to verify that are being
followed by manufacturer/ repair shop
2. Request that final reports include the technical details are include in it by
manufacturer/ repair shop
3. Increase supervision during final torque of parts
4. Avoid final adjustments to be done during night shift even for time tables
acceleration
