Women in Extended Postpartum Period

Objective : The objective of this study is to examine status of use of the contraceptive methods and factors determining their use by women in the
extended post partum period.

Background : The extended postpartum period is the time which is usually given less emphasis regarding family planning by policy makers,
service providers and users. However, it is a time with a rising risk of unwanted conception and an often-frustrated desire for contraceptive
protection. The contraceptive behavior of women in the extended postpartum period is usually different from other times in a woman's life cycle due
to breast feeding, postpartum amenorrhea, postpartum sexual abstinence or concerns of women on the effect of contraceptives on breast milk.
Understanding the contraceptive behaviors, needs and preferences of women in this period is important to design an effective family planning
program in the postpartum period.

Methodology :
A community based cross sectional study was conducted in Vyara block of district Tapi, Gujarat. All the PHCs of the block were included in the
study taking two villages from each PHC.A total of 105 women who were in the first year after delivery were interviewed using a structured data. (It
was entered, and analyzed using SPSS version 12.)

Findings : The prevalence of modern contraceptive use was found 40 percent among women in the extended postpartum period. This study
shows that women in the extended postpartum period preferred using long term family planning methods (namely tubectomy) more than any other
method. Twenty five (59.5%) of the total contraceptive users had adopted sterilization (tubectomy) as the method for family planning. It was seen
that 60 percent women were not using any modern contraceptive method with half of them quoting fear of side effects and lack of awareness as
main reasons for non-use of contraceptives in extended postpartum period. In this study, 40 percent of currently breastfeeding women were
practicing a modern contraceptive method while 60 percent were not using any modern method of contraception.

Recommendations : Number of living children, resumption of sexual intercourse and counseling during the postpartum period were found to have
significant association with use of family planning methods in the extended postpartum period. The majority of the study participants have had ANC
care, counseling, institutional delivery and postnatal counseling which are potential times for convincing women to use contraceptives in extended
post partum period. The findings of this study show the importance of the postpartum period in relation to postpartum family planning, lactation and
postpartum amenorrhea.

The objective of this study is to examine status of use of the contraceptive methods and factors determining their use by women in the extended post
partum period.
