Q1:The diagnosis of this case is: symptomatic irreversible pulpitis with symptomatic apical

periodontitis.

patient 22 years old ,male , came to the dental clinic at 10am complaining of severe and persistent
pain and he couldn’t sleep all night from that pain and the pain was spontaneous.. After taking
dental and medical history and doing clinical and radiographic examination we found deep recurrent
caries under failed composite restoration on the maxillary right first molar.

The dental history of the affected teeth:

The patient suffers of severe and persistent pain on the maxillary right first molar, he complained of
sensitivity to both cold and hot liquids and feels discomfort when eating, chewing or biting without
any stimulus.

Signs & Symptoms:

Severe spontaneous pain ,sharp

Tests:

Thermal test or electric test: positive

Percussion and palpation: positive

X-ray ‘periapical’: widening of the pdl

Treatment options:

Root canal treatment is indicated.
