VI. INSTRUCTIONAL PHASES Learning Activities

1) ACTIVATE The teacher will instruct the pupils to bring the materials for the physical activities.
2) ACQUIRE A. Activity Proper
The teacher will instruct the pupils to go to their assigned groups and perform the
given activity. Safety precautions will also be reminded to the students prior to the
conduct of the activity. Activity sheets will be distributed to each group to accomplish.

ACTIVITY 1
“How safe are you?”
I. OBJECTIVE:
1. Practice safely measures in doing physical activities.
II. MATERIALS: Copy of the observation sheet, ball pen, 2 pcs.1/4 illustration board,
pentel pen, ruler, cellular phone with camera (optional)

III. PROCEDURE:
1. Work in groups. Copy the observation sheet below your notebook. Go around the
school grounds and observe.
2. Copy or list down the messages in the signage or environmental prints that show
warning or reminders for you to follow to stay safe in school.
3. Observe your schoolmates. Are they following what was written in the signage?
Take pictures of the pupils who are following the message on the signage and take
pictures of those who are not following it if you have an available cellphone with
camera.

B. Presentation of Outputs
1. After the activity, the students will present and explain their outputs by group.
2. The teacher will use the rubrics on presentation of outputs.

3. APPLY, PRACTICE, FORMATIVE In their groups, pupils will formulate set of questions that they will ask their prospect
ASSESSMENT interviewee. The questions must focus on determining if they are safe in performing
physical activities at their respective home.
Sample question would be:

b) What reminders do your parents tell you when doing the said activity?
