DMD Serial Number is Required for Warranty Claims

Effective April 1st 2012, all warranty claims filed that includes part number 276P595010 will also require
the DMD IC serial number.
There will be no reimbursement for parts if the serial number is missing from the claim. Any approved
labor may still be covered.

Serial Number is located below 2D Matrix In this example the serial number is:
code and is 15 characters long 05Z8VLA 131106E

To properly submit the warranty claim, complete the part number and invoice as normal, then add a 2nd
part and use part number (DMD-SERIAL), invoice number will be the serial number, cost will be zero.
