AFFIDAVIT OF INTENT AND CONSENT

I, ELSA MARIE T. OLDENBURG, Filipino citizen, of legal age,

single and presently residing at 61 Junquera St., Cebu City,
Philippines, having been duly sworn in accordance with law, hereby
depose and state:
1. That I am the legitimate sister and legal guardian of ANNA TIU
OLDENBURG who was born on 23 June 2011 in Cebu City,
Philippines.
2. That I have allowed GOTHEL ALAS BITUIN, the godmother of
said minor to exercise custody of my sister ANNA TIU
OLDENBURG since the untimely death of our parents AGNARR
OLDENBURG and IDUNA OLDENBURG in a vehicular accident
last 10 January 2019.
3. That I am unable to support said child and that believing that it
would be for her own interest and benefit, I hereby signify my
written intent, freely and voluntarily given, to have my sister
ANNA TIU OLDENBURG be adopted.
4. That I also signify my written consent, freely and
voluntarily given, to have my sister ANNA TIU OLDENBURG be
legally adopted by the her godmother GOTHEL ALAS BITUIN of
Paseo San Miguel, Green Plains Subdivision, Brgy. Banilad,
Cebu City in accordance with law.
5. That I am ready and willing to abdicate my substitute parental
authority over said child in favor of GOTHEL ALAS BITUIN, who
is 30 years of age, and who is possessed of the necessary civil
and legal rights as well as the financial capacity to raise, rear and
support said child, after the adoption proceeding had been
instituted and an order of adoption be issued.
6. That I am executing this affidavit to declare my intention and
consent to the adoption of my sister ANNA TIU OLDENBURG as
well as to the truth of the foregoing facts.
IN WITNESS WHEREOF, I have hereunto affixed my signature
this 20th day of May 2020 in the City of Cebu Philippines.
