AFFIDAVIT OF UNDERTAKING

I, PARWINDER SINGH MALHI, of legal age, single, Indian citizen with residence at
Executive Village, Brgy. Tugos, West District, Sorsogon City after having been sworn in
accordance with the law hereby depose and state that:

1. I am a driver of a motor vehicle with plate no. 053710 which figured in an accident;

2. Around 6:48 in the morning of May 07, 2020, I was a party to an accident whereby I
accidently bumped the rear portion of a car driven by a certain Robert Labalan along
the highway of Brgy. Balogo, East District, Sorsogon City;

3. Because of such incident, the vehicle that I was driving was impounded by the
authorities;

4. I hereby undertake to produce said vehicle upon request of the authorities if such
will be needed for further proceedings or investigation;

5. This affidavit is being executed to support my request for the release of the
impounded vehicle;

6. I am executing this undertaking to attest to the truth of the foregoing.

IN WITNESS WHEREFORE, I have hereunto set my hands this ______ day of May,
