ANNEXURE – I

SCOPE OF WORK

1. MS & SS PROCES EQUIPMENTS, BOUGHTOUTS unloading, leading, installation and alignment and MS & SS
304 and 316 PIPING & ducting fabrication, welding, erection, supporting, finishing, painting and clamping etc

2. You should arrange all skilled & Unskilled Labor/Manpower, project manager, technical team (Mechanical engineer),
site office, and fabrication shed as per requirement, Machinery, Consumables, tools and tackles, Scaffolding for etc.
for specific work with different work force and machinery.
3. Scope of work & activity to be carry with respect to different sections.
SCOPE

o Required drawings & raw material of MS & SS for construction will be provided.
o MS & SS process equipments & BO will be provided for erection.
o Water will be provided to carry work and testing.
o Single electrical point, water point and electricity for fabrication, welding and testing.
o Space for labor hunt /accommodations.

CONTRACTOR SCOPE

 EQUIPMENT & BOUGHT OUTS - Work involves Shifting, Unloading, Erection, Alignment & Assembling of
Fabricated SS & MS process equipments with site joint welding, flange joint, grinding, and proper finishing
with acid / k2 paste, equipment staircase, railing, ladder, platform and testing and commissioning till leak
proof. Bought outs erection and alignment work like Pumps & motor (Coupling alignment work involves),
PHE, blowers, air compressor, control valves, valves, etc.and as per the drawings and under the guidance &
advices of our project manager.

 MS & SS PIPING & DUCTING - Work involves Shifting, Unloading, Erection & Fabrication, Alignment &
Assembling of MS & SS pipes, welding of pipes, Nitrogen purging for SS pipe, fabrication of Pipes, elbows,
and reducers etc.. Erection of all instruments valves mentioned in drawing, and all fittings, Supporting &
Clamping, Acid cleaning and testing and commissioning till leak proof and as per the drawings and under the
guidance & advices of our project manager. Piping measurement will be done in inch dia meter, as there will
no separate bills for bend, flange, Control Valve, Eccentric and Concentric reducer, Valve and Stub bend.

 You shall arrange all required machinery like Crane, Paraná, Hydra, winch, welding & grinding
machines, hydraulic Jack, Screw jack etc.. With respect to work and item sizes. All your machinery should be
approved by our site engineers before carrying the work.
 Where ever scaffolding requires must be arrange by contractor for positing welding and duct erection work

 Required manpower & machine should be arranged for Unloading of material in respective yard, shifting of
material to place of fabrication and as per the drawings and under the guidance & advices of our project
manager.
 Man power for arranging store material segregation stacking in storage area.
 Contractor should provide issue slip to store keeper for receive material.

 The scope also is inclusive of transportation of your machineries & manpower, lodging & boarding, insurance,
workmen compensation for your labours and engineers.

 You shall comply with all the statutory legislation pertaining to construction, like site safety, labour safety,
environmental clearances, work licence, tax registration, any other related clearance required to carry out
contractor job for this project.

 You shall get your welders approval done by our project manager/QC at project site before commencement
of welding work.
.
 You must submit the material account on weekly basis. New items / accessories will not be issued to you
unless you have submitted account of old material and reconcile. You will return the scrap and unused
materials to our stores. Responsibility of watch and ward and safety of materials issued to you lies with you.
 You shall submit an inspection report against the tolerances specified by us respect to proceed for further
activity. You must submit the work progress report compiling of all sections as per our required format which
inclusive of manpower & machine strength and material account on daily & weekly basis respectively. New
items / accessories will not be issued to you unless you have submitted account for completed job.
 You must arrange and allocate sufficient manpower and machineries with respect allocated quantum of work
and as per requirement of site engineers/ managers advice and submit the report on daily basis.
 Target for the quantum of work to be executed by you shall be approved and decided by our site coordinator /
project manager / Engineer on weekly basis. This target shall be used as yardstick to evaluate your
performance. You shall submit weekly work schedule/planning against our project schedule.
 You will provide sufficient manpower for Pre-commissioning checks for all sections of work, if any defects
found will rectify all the defects / leakages etc. at your own cost and arrangements shall be made by you &
shall be done all testing in presence of our site engineer/manager.
 Water and Electricity will be supplied on site at one point free of charge; contractor will make his own
arrangements for taking water, storage & electricity at required Location as per requirements, & make optimum
use of this without any unnecessary wastage.
 Accommodation and food to your engineers/managers and labors lies with you until work completion. Space
for labour hunts will be provided by NGEP.
 Additional materials shall not be provided by us for any additional supporting work carried as scaffolding,
internal supports for work, bottom plate, structure straightning, for erection of equipments, usage to
machineries, cranes. Each section supporting work to be carried by using respective materials without any
damage/scrap. If damage/scrap found the same will debited to contractor.
 Any kind of Scaffolding, Structure and supporting structure required for the fabrication, erection, welding (All
type of welding) and complete finishing up of the structure shall be arranged by Contractor..
 You must follow the engineering standards for fabrication & erection and followed as per drawings and
 Advices of our project manager/engineers.

Annexure – II

INSPECTION PROCEDURE

STAGES OF INSPECTION AS APPLICABLE FOR ACTIVITY TYPE

1. Visual Inspection of raw material before fabrication and layout marking stage (Dimensional Check)
2. Dimensional Check after cutting (Diagonal & Length) and Edge preparation of plate (Visual checking)
3. Dimensional Checking after tacking (Plumb, diagonals plumb, gap, offset etc.) before release for welding.
4. Visual Inspection of cleaning before releasing for welding, after root run and after final welding.
5. Back chipping up to sound metal and D.P. Test.
6. Visual Inspection after final welding (Back welding)
7. Marking and Setup (Orientation, Elevation, Size etc.)
8. Dimensional check after welding (diagonals, Straightness, height / length plumb etc.)
9. Final Inspection after grinding and finishing smooth (before lining/ painting / coating)
10. Final dimensional inspection.
11. Straightening is required before SS Pipe Welding to avoid pipe line bending Problem.
12. For Structure Primer and Finish application must done by roller, Emery Paper, brush for spatter cleaning.

Under the guidance and advices of our project manager.

NOTE:

1. All above stages will be checked by our QC inspector/engineer and released only after getting clearance in
writing for one stage you will be allowed to do further work.
2. All SS & MS welders are to be qualified by our TUV Engineer and only qualified welders are to be used for
tacking and full welding.
3. At the time of Water trial total team required for nut bolt tightening , Packing, Pump suction Mesh fitting
Welding leakage arresting
4. Structure, Equipment, Tank & SS & MS Piping fabrication & erection shall be done under TUV Inspection.

Annexure – III

CONTRACT PRICE, PAYMENT TERMS, BILLING SCHEDULE AND MODE OF MEASURMENTS

Contract Price & Payment Terms:

1. Approximate total item rate contract value will be ------------- subject to actual work completed at site.
2. Completion period will be -------- days from date of issue of work orders. All the rates are fixed and firm till the
completion of the project.
3. Both parties have agreed for work quantum per lot unit of measure for specific work covering Project. The rates
shall remain unchanged till completion of mentioned work.
4. Completion period will be inclusive of Mobilization and any Holidays and unforeseen issues.
5. If any work hold by Excel due to erection/ other reasons suitable time extension will be given. This will overrule
Clause as job completion time will be extended accordingly.
6. In case of failure to achieve the targeted schedule of assigned work or lack of manpower or mismanagement
conducted by the contractor, Excel has the rights to divert/allocate the works to the other/different contractors
without any permission of the Contractor and there after accordingly amendment of respective work order will be
issued and the same should be accepted by Contractor without any legal implications.

Payment Terms:

 10% as advance of total contract estimated Basic amount would be released one week after submission of
100% site mobilization report by our Site engineer / Manager duly signed with respect to said manpower,
machineries (Ref Annex I list ) & hydra.
 80% of Each RA Bill will paid within 7 to 10 working days after Submission of bill, in prorate basis and
evidenced by receipt of invoice and the Progress Certification from Excel site manager. Monthly RA bills to be
th
submitted by 25 of every month.
 10% will be kept as retention and will be paid after 6 months of successful commissioning and submission of
final bill duly certified by site manager.

Mode of Measurement:

All the work executed will be measure as per items mentioned in the schedule of rates. For all works joints
measurements will be taken by contractor with our Site engineer / Site in charge time to time & record will be
maintained by contractor in duplicate book.

If any dispute arises while measurements, then IS standard mode of measurement will be considered as
Standard one and will be settled in PUNE jurisdiction.

Annexure - IV

A. GENERAL TERMS AND CONDITIONS:

A.1 Deduction towards Income Tax shall be effected from your payment as per the prevailing rules and regulations. A
certificate shall be issued to you showing the deductions so made.

A.2 You shall comply with all statutory requirements and wherever applicable e.g. Workmen’s Compensation act, Minimum
Wages Act, Insurance Cover, ESIC, Provident Fund etc., for your work force (Staff / Workmen). Copy of the same to be
submitted to our office for our information & records.

A.3 Contractor is solely responsible for workers safety at site. Contractor has to provide safety equipment’s like safety shoes,
safety belts, safety helmets etc., at site.

A.4 If you default in paying wages / any compensation / compliance of statutory payments to your workers or other, we
reserve the right to deduct / collect such amounts from your bill or bank Guarantee as may be deemed fit to fulfil such
obligation on your behalf.

A.5 All the rates for works are Inclusive of Transportation, or levies, loading/unloading, shifting, mobilization /de mobilization
etc.

A.6 The entire rate for works is excluded of Taxes & duties.

A.7 Daily site report, having amount of work completed in Kg/MT & Sq.Mtr , measurements if any, and remarks to be
maintained properly along with daily manpower report.

A.8 Discipline at site, housekeeping and cleanliness should be observed.

A.9 We wish to make it clear that ‘SAFETY FIRST’ is the paramount importance for the job being assigned to you. There are
no short cuts or unsafe practices to be carried out by your staff.

A.10 Child labour will not been allowed/engaged at any stage and you will abide the laws of Indian government in regards to
the child labor and women labour Act.

A.12 Labor hutment and labour food will be in contractor scope if customer not provided.

A.13 General Terms and Condition which is not mention here with as per good engineering practice is deemed to be including
and must be complied with during execution of the order.

A.14 No Idle Charges of labour and machineries will be paid.

A.15 No escalation on rates due to any cause till the completion of the project.

B. PENALTIES FOR WASTAGE OF MATERIAL AND TARGET AND QUALITY

B.1 If the work will not complete within the above said period Excel will have rights to apply the penalties @0.5% per week of
delay subject to a maximum of 5% of contract value for reasons solely attributed to contractor.

B.2 The quality of workmanship will be assessed strictly as per the ISI Standard.

B.3 If a portion of the work is found defective or not meet up the require standard by the site representative, the entire cost of
that unacceptable work together with 10% of total cost as our overheads shall be recovered from you immediately from
the next running bill payable to you.

B.4 Reconciliation of consumption of Pipes, Plates and structural Steel is to be maintained by you. And to be furnished to our
technical staff periodically.

B.5 All manpower, machinery, tools and tackles, testing equipment shall be arranged by you and will confirm to relevant IS
Standards and Best Quality available.

B.6 Progress of work should not be held up due to shortage / non-availability of raw materials for items.

C. LIQUIDATED DAMAGES FOR DELAYED COMPLETION

C.1 We reserve the right to recover from you by way of liquidated damages for any direct or indirect loss incurred by us due
to non-completion of work within the targeted completion period at the rate of 0.5 % per week of the order value subject
to a maximum of 5% value of the order.

D. ARRANGEMENT TO BE MADE BY CONTRACTOR

D.1 You shall make arrangement for the provision of technical supervision, skilled and unskilled manpower requires carrying
out work. You have to get this approval within 3 days from accepting this contract.

D.2 You shall use only those materials which are approved by our Site In charge for the job. If your person is found using
non-approved materials on job, Site Co-ordinator / Site Engineer will have complete authority to reject the job and
recover all direct and indirect cost incurred in that item of work.

D.3 You shall ensure compliance with discipline and security regulations applicable by Excel.

D.4 You shall submit weekly progress report of the work done and also daily strength of labour force category wise employed
at the project site to our site representative.

D.5 Progress for next day should be decided and finalized with our site representative by you every day in the evening.

D.6 Any person found to be unsuitable to be employed at site shall be removed from the Premises immediately after
intimation received from our site in-charge

Annexure – V

GUIDE LINES FOR CONTRACTOR AS PER ISO-14001

A) INSTRUCTIONS TO BE FOLLOWED BEFORE EXECUTION:-


1) Gate pass for workers shall be obtained from competent authority.
2) Work permit shall be taken from relevant plant in charge before commencement of work.
3) Requirement of fireman/safety engineer if mentioned in the work permit, the presence of the same at work place shall
be ensured.
4) Proper scaffolding shall be prepared as per site requirement.
5) Restrict the spreading of sparks by covering area with asbestos clothes, wherever required as per work permit.
6) Required material brought at site shall be stored properly at safe location.
7) Hazardous material if any shall be kept at safe location/allocated area with required safety Precautions.
8) Required electrical connection to be made with isolation & safe protection device.
9) Open drain is to be covered near workplace wherever required.
10) Cordoning of required area shall be carried out before any radiography work at site.
11) WC required for Manpower & Company ID required for total Manpower.

B) INSTRUCTIONS TO BE FOLLOWED DURING EXECUTION:-


1) Workers must wear safety helmet, hand gloves etc. as per safety requisite.
2) Permit must be renewed in every shift from plant in charge as per issued work permit.
3) Any leakage's occurs during work shall be immediately informed to safety dept. /plant.
4) Heavy equipment dismantling & removal shall be done with appropriate tools/tackles including crane.
5) Removed equipment/insulation/misc. material shall be kept at specified location.
6) Required electrical connections to be made with isolation and safe protection device.
7) Welding buds, used grinding wheels, scrap generated due to welding/cutting, packing material, waste insulating
material, empty containers of DP test, scrap cables, PVC tubes, cotton waste etc. shall be collected and disposed to
scrap pit.
8) Gas cylinder used for welding/cutting operation shall always be stored in vertical position.
9) During grinding, air blowing, gasket bursting, pneumatic breaking and other noise generating activity, ear buds shall
be used.
13) Area of work during excavation, radiography, sand blasting shall be cordoned with warning tags of "work in progress”,
“no entry”, “radiography” in progress' etc.
14) Spillage of oil if any on land during transformer charging and collected in shall be recovered from pit and handed over to
concern group.

C) INSTRUCTIONS TO BE FOLLOWED AFTER EXECUTION:-


1) Site shall be cleared from any left out scrap material/waste after completion of work. Balance useable material shall
be return back to store/relevant place.
2) Any old recovered useable equipment/material shall be return back to store after thorough cleaning by operation
group.
3) Record of material shall be taken care of for reconciliation of material after job completion.

SAFETY GUIDE LINES/RULES FOR CONTRACTORS & OTHER WORKERS

In case of various maintenance & modification activities number of hazards are involved. The following are some of the
hazards that may be encountered:

Over head hazards, moving cranes etc.
Radioactive substances/sources
Confined space hazards entry
Hand tools/portable tools hazards

As mentioned above all persons involved in job will have to look out for hazard & to take necessary actions so that accident
can be prevented. The following are some general guidelines & check points that should be followed:

1. Unauthorized entry into any battery limit of plant is strictly prohibited.

2. It is forbidden to take children & under age workers inside the factory.

3. Smoking & carrying matchbox, cigarettes, lighter etc. are prohibited.

4. Photography & carrying cameras is strictly prohibited in all areas.

5. Reckless driving or other non-observance of traffic safety rules shall result into withdrawal of permission to carry vehicles in
side factory.

06. The maximum speed limit is 20 Kms. /hour. However, much slower speed is necessary in congested areas.

7. When you are given permission to enter in Dalmia Distillery complex you are to go only to the location, which you have
been authorized. Don’t visit or loiter in any other plants/premises. When existing the plant, you have to leave through the
gate you entered, unless otherwise directed, in case of emergency.

8. All necessary personal protective equipment (PPEs.), Safety equipment should be made available to use for persons
employed by the contractors on the site &shall be maintained in condition suitable for immediate use. Protective
equipment for head protection, body protection, eye protection, hand protection, hearing protection & respiratory
protection shall be made available by the contractor.

9. Use of hoisting machines & tackle including their attachments, anchorage & supports shall conform to the following
standards or conditions: -

a) These shall be good of mechanical construction, sound materials & adequate strength & free from patent defect &
shall be kept in good condition & in good working order.

b) Every rope used in hoisting or lowering materials or as a means of suspension shall be of durable quality &
adequate strength & free from patent defects.

c) Every crane driver or hoisting appliance operator shall be properly qualified and no person under the age of 21
years should be in charge of any hoisting machine including any scaffolding, watch or give signals to the
operator.

d) In case of every hoisting machine & of every chain ring hook, shackle swivel & pulley block used in hoisting or
lowering or as means of suspension, the safe working load shall be ascertained by adequate means. Every
hoisting machine & all gear referred to above shall be plainly marked with the safe working load & the conditions
under which it is applicable shall be clearly indicated. No part of any machine or any gear referred to above in this
paragraph shall be loaded beyond the safe working load except for the purpose of testing.

e) In case departmental machine, the safe working load shall be notified by the Engineer-in-charge. As regards
Contractor’s machines, the contractor shall notify the safe working load of the machine to the Engineer-in-charge,
whenever he brings any machinery to site of work & get verified by the Engineer-In-charge concerned.

13. Motors, gearing, transmission, electrical wiring & other dangerous parts of hoisting appliances shall be provided with
sufficient safe guard hoisting appliance should be provided with such means as will reduce to the minimum the risk of
accidental descent of the load. Adequate precautions shall be taken to reduce to the minimum the risk of any part of a
suspended load becoming accidentally displaced. When workers are employed on electrical installations, which are
already energized, insulating mats, wearing apparel. Such as gloves & boots as may be necessary shall be provided.
The workers shall not wear any rings, watches & carry keys or other materials, which are good conductors of
electricity.

11. Don’t check gas leaks with lighter, matches or other flame.

12. Don’t tamper with any type of electric switches-equipment or any other appliances or moving machinery installed in
factory area.

13. Don’t operate any switch, valve etc., without permission.

14. No welding/grinding/cutting/soldering or open flare/fire etc. should be done without valid safety permit issued by the
Engineer-In-charge. While welding/grinding/cutting make sure that sparks & molten slags etc. don’t fly or come into
contact with combustible materials surrounding equipments, valves etc. i.e. make provision for collection of sparks.
15. Always keep gas cylinders away from direct rays of sun, hot place, and welding, grinding & cutting sparks.

16. Valves on cylinders should not be lubricated.

17. Gas cylinders should be kept away from work place & Acetylene cylinders shoul be kept vertical.

18. Cylinder should not be rolled on roads for transportation from stores or one place to another place, use suitable handcart
for the purpose.

19. It is prohibited to carry gas cylinder up-stair in the plant or in-side the vessel or confined spaces for cutting-welding job.

20. Electrical equipment should be considered live unless it is ensured that they are isolated & made dead.

21. All the electrical equipments should be properly earthed as per Indian Electricity Rules.

22. be careful to keep all aisles, passageways and stairways clean & unobstructed. All discarded metal & other scrap should
be collected.

23. Storage area for keeping equipments, machines & other raw materials should be isolated & properly protected.

24. It is forbidden to remove guards from moving machinery or to clean/check moving parts of machines while in motion.

25. Before commencing repairs on a machine main power supply to it should be cutoff & locked. Display cautionary tags on
starting switches. Before you start make sure that everything is in the clear. Guards/covers should be put in position.

26. Any person without obtaining prior permission from the concerned supervisor should undertake no works on any
machinery/equipment/piping.

27. It is prohibited to work on or near moving machinery by wearing loose clothing.

28. Nobody should try to clean, lubricate any part of the machinery while the machine is in running.

29. Welding cables & connections should be in good condition & fix the earthing cables at the welding place only.

30. No electrical connection should be done without suitable fixtures such as plug pin, switchboard, ELCB, fuses etc.

31. Starting of any machines or power tools or turning on steam/power or gas should be done only after ensuring that
nobody is in danger.

32. Sitting or walking on rail tracks, crossing between wagons, taking rest under stabled wagons, crossing the rail through

33. When work has to be done on elevated places, towers, roofs, pipe racks & other lofty positions where plat-forms & other
fall guards are not there, use of SAFETY BELT is compulsory. Safety Nets will prove very helpful in case somebody
slipped from height.

34. Safety helmet must be worn. Nobody shall enter into site without wearing safety helmet.

35. Standing under a suspended load is very dangerous. It may slip & fall-on you there by causing serious injury & even
death.

36. All the sewers or openings/cut-outs should be kept covered to avoid pit falls.

37. Use of SAFETY APPLIANCES like safety goggles, canvas hand gloves, welding helmet, chrome-leather hand gloves,
safety shoes, etc. during welding / chipping / grinding should be enforced.

38. Combustible materials like wooden pieces, cotton waste, bags etc. should be immediately removed to safe places.

39. Use of ‘Fire Blankets’ in order to prevent flying of welding/cutting sparks & contacting with surrounding area should be
ensured.

40. Welding cable should be examined & all open wires should be properly insulated.

41. Hands should be thoroughly washed before touching anything that goes in your mouth.

42. All concerned personnel at site should maintain a high standard of Cleanliness’.

43. The ‘Safety Appliances’ supplied to the personnel should be used while doing the job & be maintained in good working
conditions. The misuse of safety appliances should be prevented.

44. Permission of a supervisor before any excavation is a must. Also the presence of underground electric cables or any
pipelines must be taken care of during excavation.

45. Excavated earth must not be dumped within five feet. The further the better.

46. Passages/Bridges across trenches should be provided with hand rails & toe board.

47. Entry permit should be obtained while entering a pit with a depth of more than one meter.

48. No pit should be left unguarded / uncovered. Provide proper barriers around the pit when persons are not working in it.
Red illuminated signal should be displayed so that nobody goes near the pit during dark hours.

49. Only limited person should be allowed to enter in pit for digging or dumping of excavated earth when digging is going on.
Two works should not be carried out simultaneously. Also before starting digging or dumping proper tools should be used
during digging & dumping operation.

50. Proper arrangement of ladder should be made to enter into the pit.
51. No work, however, small should be under taken/started without obtaining valid work permit from the concerned
department.

52. Confined space entry should be done only by valid entry permit from the Engineer-in-charge.

53. Proper approaches/scaffoldings/ladders etc. must be provided to avoid falls.

SAFETY GUIDE LINES/RULES FOR ELECTRICAL SAFETY

1. Each machine shall have power supply isolation switch in IP-55 enclosure & minimum 30 meters lengths of 10 mm2 Cu
flex wire with PVC insulation.

2. Isolation switch shall have HRC fuses of suitable ratings (32A).

3. All switches, machine & regulators are to be earthed properly with earthing bus.

4. Outlets of welding machine (i.e. holder cable and return earth cable) are to be laid up to job and return cable should be
connected firmly with job.

5. All power cable connections and joints are to be made through lugs, to avoid heating & fire.

6. Each electrically operated machine shall have earth leakage operated MCB to isolate the power in case of electric shock to
human.

7. Regulator extensions are to be connected through 3-pin plug socket.

8. Contractor must get their machine examined from respective elect. Group and insulation resistance value of machine &
their associated accessories should be maintaining more than 2 ohm.
