Industrial attachment report structure
The Industrial Attachment Report is aimed at assessing how the student relates theory to
practice. It should be in 11/2 line spacing Times New Roman font size 12 (except headings) and
spiral bound.

1. Cover page - University name, Name of School, Name of Department, University logo,
candidate name and registration number in brackets, Purpose and submission date of the
Report, name of organization
2. Preliminary pages
i) Declaration
ii) Acknowledgement
iii) Abstract showing purpose of the research, major objectives, major observations,
major conclusions and major recommendations
iv) Table of contents showing major items of the report and the pages they cover.
3. Chapter 1 background
1.1 Introduction
1.2 Location (map required)
1.3 Vision, mission statement and core values of firm.
1.4 Objectives of the firm
1.5 Organizational structure
1.6 Brief description of department(s) you worked in
1.7 Organization’s business, target market, company performance analysis
4. Chapter 2 Industrial attachment activities
Field attachment activities; reporting on work actually carried out by the student
5. Chapter 3 Linking theory with practice
Discussion of experience and knowledge gained by the student in relation to theory and
practice. Student should attempt to answer the following questions:
3.1 Is theory agreeing or disagreeing with practice
3.2 Is theory up to date, applicable, adaptable and acceptable
6. Chapter 4 Conclusions and recommendations
4.1 problems encountered during the program
3.3 4.2 Suggestions for improvement to the organization and the university
7. Appendices
All other supporting documents are included as numbered appendices, such as a list of
References, Organizational photos, organizational catalogues etc.
