ONLINE ENROLMENT PROCEDURES

SY 2020-2021

FOR GRADE 7

1. Secure a soft copy of Learner Enrolment and Survey Form (LESF) from Enrolment Focal Person.
2. Coordinate with the Enrolment Focal Person to secure the link and password for the online enrolment.
3. After securing the link and password, proceed and fill up the data needed in the ONLINE ENROLMENT.
4. After filling up the data, upload the following needed files:
a. Form 138A/Elementary Card (optional)
b. 2x2 Picture (Required)
c. Learner Enrolment and Survey Form (Required)
5. Click submit (your enrolment application will be processed & verified by the Grade 7 teachers of DCSNHS)
6. After VERIFICATION, a notice will be sent to your email or contact number regarding the status of your
online enrolment application.

FOR HIGHER YEARS (GRADES 8 TO 12)

1. Secure a soft copy of Learner Enrolment and Survey Form (LESF) from your previous advisers.
2. Email the following to your current section adviser (sections already posted in the Official FB Page of the
School):
a. Learner Enrolment and Survey Form (Fully Filled Up)
b. Proof of payment of PTA obligations
3. You will be notified through email that you are officially ENROLLED in the school for SY 2020-2021 .

FOR LSEN (LEARNER WITH SPECIAL NEEDS)

1. Arrange for an online virtual interview with the LSEN Coordinator Ms. Daisy C. Sanchez.
2. Present the Behavioral/Developmental Doctor’s assessment during the virtual interview for validation.
3. After the online validation, you will be redirected to Mrs. Hyacincth C. Belches for the online enrolment.
1. Secure an online link from Mrs. Belches to access the Online Enrolment Form for LSEN.
2. Completely Fill Up the Information in the Online Enrolment Form.
3. Upload the following documents in the online enrolment form:
a. Learner Enrolment and Survey Form (LESF)(Required)
b. Form 138A/Elementary Card (optional)
c. 2x2 Picture (Required)
d. Assessment Form duly signed by accredited Developmental/Behavioral Doctors (Required)
4. After uploading and submitting the documents, you will receive a notification signifying approval of your
enrolment.
