Dear Potential Donor,


Thank you for your interest in bequeathing your body after death for the National Repository
Centre. For your information, you may like to know that the bequests accepted by the Centre
are used for the education and training of student doctors and health care professionals and are
not used for research into specific diseases or illness.

Please find attached an information pack, which fully explains the position regarding the
bequeathing of remains to the Repository Centre. I’d recommend that you read the information
carefully and then keep the pack in a safe place (e.g. with your personal papers, your Will, or
with your family doctor or solicitor). It is also recommended that you inform your next-of-kin
and/or executor(s) and your GP of your intention to donate your body, as it is important that
whoever will be responsible for your affairs when you die knows about the bequest papers and
where they are to be found.

You might also like to note that there is no upper age limit for bequeathing a body.

May I stress that we do endeavour to fulfil every bequest but it is important for you to realise
that we cannot guarantee to accept your body on your death. The main reasons for this would
be if there were any infectious agents or deterioration in your body tissues at the time of death.
Therefore, I would recommend that you do consider making some alternative funeral
arrangements just in case it is not possible for us to accept your bequest when the time comes.

If it is possible to accept your bequest after your death, then the Centre will undertake to
arrange and pay for a cremation. However, you next-of-kin/executor will be asked to contribute
towards the cost of transporting your body to the Nottingham Repository Centre (maximum
£400).

If you would like to proceed with donating your remains to the Repository Centre, you will need
to complete both copies of the Bequest Consent Form in the presence of a witness (this may
be a spouse/partner, next-of-kin, executor, friend, GP, solicitor, etc.). One copy should be
returned to me so that your name can be included in our register of potential donors. The
second copy should be retained by you and kept in a safe place (e.g. with your personal papers,
your Will, or with your family doctor of solicitor).

Please also complete the Donor’s Medical History Form and send this back to us with your
Consent Form.

The Centre would like to thank you most sincerely for your kindness in thinking of making this
decision, since the training of future doctors depends upon generosity of this type.
