 Responsible, cooperative and supportive RHU staff with harmonious relationship

 Competent and dedicated NDP nurses and BHWs

 SERVICE DELIVERY

 Accessible health services for all population groups – OPD, TB DOTS, prenatal meds,
maintenance medications and supplements, first aid and emergency medications

 Practice of continuity of care wherein patients are motivated to follow up which also
includes home visits for known cases

 INFORMATION AND RESEARCH

 Organized patient charts that are arranged per household (profiling)

 MEDICAL PRODUCTS, VACCINES AND TECHNOLOGIES

 Free medicines from DOH

 Augmentation of medical supplies by occasional donations from private entities

 Presence of free vaccines for children

WEAKNESSESS

 LEADERSHIP AND GOVERNANCE

 MHO lacks consistency in monitoring and organizing tasks for logistics, operations,
planning and evaluation

 BHWs were not able to comply to and complete their designated tasks

 HEALTH WORKFORCE

 No regular staff meeting

 Understaff- NDP nurse assigned holds more than 1 barangay

 Inadequate training of BHWs

 SERVICE DELIVERY
 Varied efficiency of service delivery in the barangay level

 INFORMATION AND RESEARCH

 Individual patient information and charts are not computerized

 Statistics and census are not up to date

 MEDICAL PRODUCTS, VACCINES AND TECHNOLOGIES

 Delayed procurement of free medical products which is sometimes due to late


submission of reports
 Medical tools and paraphernalia such as BP app, stethoscope and weighing scale are not
readily available

OPPORTUNITIES

 New ongoing health agendas of the newly designated nurse


 Motivated, supportive, and hard-working BHWs
 Health-seeking behavior of residents and their awareness and willingness to be part of programs
dedicated for their community

THREATS

 Some members of the healthcare system are not fully participating in the activities
