SUBJECT : Investigation Report on Person Under Monitoring Relative to

COVID19 Outbreak

DATE : April 21, 2020

I. AUTHORITY:

II. MATTERS TO BE INVESTIGATED:

2. To determine the circumstances surrounding the unauthorized

travel of retired PINSP Atonio G Guban.

3. To determine and to know the premises surrounding this

particular incident re arrival of retired PINSP Antonio Guban y Gratila, 68
years old and a resident of Purok 1, Brgy. Ibingay, Masbate City at the
coastline of Sitio Dagat-Dagatan, Purok 1, Brgy. Tugbo, Mobo, Masbate on
board motorized boat on April 10, 2020 on or about 5:00 PM.

III. FACTS OF THE CASE:

Mr. Antonio Guban arrived at the coastline of Sitio Dagat-Dagatan,

Purok 1, Brgy. Tugbo, Mobo, Masbate on board motorized boat on April 10,
2020 on or about 5:00 PM despite of the strict implementation of Executive
Order No. 06 by the office of the Governor (An E.O. Implementing an Enhanced
Community Quarantine Over the Province of Masbate for the Management of
COVID-19 and Its Effects on the Community).

IV. INVESTIGATION/FINDINGS:

4. Based on the statement which was given by Mr. Guban the subject
person himself, he averred that on March 12, 2020, he went to Calamba,
Laguna purposely to get his newly acquired four-wheel vehicle Toyota REVO,
after which on March 14, 2020 he drove his newly acquired vehicle going to
Masbate City. However, Enhanced Community Quarantine was already in the
strict implementation in the entire Luzon due to COVID19 outbreak. Likewise,
sea transportation was deferred and the latter was not able to cross going to
Masbate Province in which he decided to go and stay in his hometown in Brgy.
Fabrica, Bulan, Sorsogon. Throughout the course of his stay on said Barangay,
he undergone 14 days of Home Quarantine as attested by the Municipal Health
Office of Bulan, Sorsogon. (copy of the Home Quarantine Certificate of
Completion of Municipal Health Office, Bulan, Sorsogon is hereto attached as
“Tab A” ready for your reference)
 5. Mr. Haji Valladores y Zurbito, a son-in-law of subject person
through cellular phone call revealed that on April 9, 2020, his wife died while
undergoing medical treatment in Philippine General Hospital for Autoimmune
Disease Systemic Lupus Ensythematosus- Induced Hepatitis. And as such, the
remain was cremated and the human ashes cremation was still in the care of
Mr. Valladores in Manila. (copy of death certificate and medical abstract are
hereto attached “as Tab B” ready for your reference)

6. On April 10, 2020, due to the death of his daughter, Mr. Guban
decided to go home in Masbate City in order to reunite and mourn together
with his family. On the same date he exerted a diligent effort to secure a Travel
Order from the Office of Municipal Mayor, Bulan, Sorsogon in order to have a
trip going to Masbate Province for emergency purposes. As indicated therein, a
Government Sea Vehicle was intended to be use on such travel. However, due
to unavailability of Government Sea Vehicle, Mr. Guban was able to look for a
private motorized fishing boat at the Port of Bulan, Sorsogon owned by a
fisherfolk from Magallanes, Sorsogon. And presentation of travel order to the
duty coast guard of the said port was religiously done by the latter as if to allow
the said motorized boat to transport him going to Masbate. (copy of travel order
is hereto attached “as Tab C” ready for your reference)

7. In the Barangay Blotter of Brgy. Tugbo, Mobo, Masbate, on April

10, 2020 on or about 5:35 PM Mr. Guban arrived at the coastline of Sitio
Dagat-Dagatan, Purok 1, Brgy. Tugbo, Mobo, Masbate on board on a motorized
boat. Subject person alighted in a motorized boat and walked to the shoreline
of said Barangay as it was seen by Melchor Bauso y Apaya, 32 years old,
married, Habal-Habal driver and a resident of said place. Thereafter, the latter
immediately look for available means of transportation that can bring him to
PC Camp Site, Brgy. Ibingay, Masbate City. And as the latter saw Mr. Melchor
Bausa, he introduced himself as a retired Police officer and stated that he came
from Brgy. Lagundi, Batuan, Masbate. Immediately, Mr. Bausa was convinced
and paid by the latter to transport him going to Masbate City. And while
passing on the Quarantine Control Point at Brgy. Tugbo, Masbate City, they
were subjected for thermal scanner by the BFP personnel with normal body
temperature result; (copies of Barangay Blotter and Barangay Certification of
Brgy. Tugbo, Mobo, Masbate are hereto attached “as Tab D”)

8. As averred by Wella Sese Espinas- Barangay Health Worker,

arrival of Mr. Guban information was reached in their office on April 11, 2020
wherein she together with Marife Espinilla Empleo- Brgy. Midwife immediately
proceeded at the house of subject person to coordinate with and advised him to
submit himself for fourteen (14) days home quarantine. Nevertheless, on April
12, 2020, based on the Barangay Blotter by Brgy. Ibingay, Masbate City, Mr.
Guban was seen outside of his residence and Mrs. Guban denied that her
husband is came from Manila. (copy of the Barangay Certification and Blotter
are hereto attached “as Tab E” ready for your reference).

V. DISCUSSION:
9. The subject person himself clearly established his travel to
Calamba, Laguna on the faithful day of March 12, 2020, that his only purpose
was to bought a vehicle, that time he never expected that the Government will
declare ECQ to entire Luzon that will caused his travel more complicated.
However, Mr. Antonio Guban decided to stayed for almost twenty-six (26) days
at Brgy. Fabrica, Bulan, Sorsogon and prior to his returned in Masbate City, he
undergone home quarantine thereat, it was attested by Municipal Health Office
of Bulan, Sorsogon. The death of his daughter prompt him to returned in
Masbate Province amid the strict implementation of Enhanced Community
Quarantine relative to COVID19 outbreak of a pandemic disease, he was able
to secured travel order from the Office of the Mayor and subsequently his
arrival in
Brgy. Tugbo, Mobo, Masbate, was witnessed by Mr. Bausa. This incident was
reported in Barangay Ibingay, subject person was advised to observed Home
Quarantine for 14 days and undergone a daily monitoring of his medical
condition. In fact, he is in good physical and medical condition, contrary to the
spreading gossip coming from the community that he has a fever. And as to the
status of the said Habal-Habal driver whom has close contact to the subject
person, a home quarantine for fourteen (14) days was strictly imposed to him
by Brgy. Chairman Dominguito Apaya of Brgy. Tugbo, Mobo, Masbate.

VI. CONCLUSION:

10. After the careful perusal of circumstances surrounding this

particular incident, subject person was confirmed arrived in Masbate Province
despite of Executive Order No. 06 by the office of the Governor (An E.O.
Implementing an Enhanced Community Quarantine Over the Province of Masbate
for the Management of COVID-19 and Its Effects on the Community), although
his travel was supported by a travel order and Home Quarantine certificate,
but the latter failed to coordinate and report his arrival to proper implementing
authority of the ECQ. The subject person arrival caused panic in the
community considering that he is classified as person under investigation, he
must be cooperative and seriously obey in dealing with his fourteen (14) days
home quarantine period.

VII. RECOMMENDATION:

11. WHEREFORE, it is recommended that this investigation

report be given a merit and scrutiny by superior officers.
